3. PHARMACOKINETICS AND PHARMACODYNAMICS:
Pharmacokinetics properties
General characteristics of the active substance:
Distribution studies performed in rats indicate minimum muscle diffusion from the complex of botulinum neurotoxin type A in the gastrocnemius muscle after the injection, followed by a fast systemic metabolism and urinary excretion. The amount of radioactive tagging material in the muscle was reduced at an average life of approximately 10 hours. In the injection site, the radioactivity joined large protein molecules, while a few amount of radioactivity reached the systemic circulation, suggesting a minimum systemic exposure. Within 24 hours after dosage, 60% of radioactivity was excreted in the urine. Probably, the toxin is metabolized by proteases and the molecular components are under cycles through normal metabolic routes.
Classic absortion, distribution, biotransformation and elimination studies have not been performed on the active substance due to this product´s nature.
Characteristics in patients:
It is believed there is systemic distribution of therapeutic doses of BOTOX®. However, the clinical trials that have used single fiber electromyographic techniques have shown mild electrophysiological effects, consisting of neuromuscular inhibition (that is, “agitation”) in muscles that are far from the injection site, but they are not accompanied by any clinical sign or symptom of neuromuscular inhibition of the botulinic toxin effects.
Pharmacodynamics properties
Mechanism of action:
The complex of neurotoxin of Clostridium botulinum Type A blocks the release of peripheral aceltylcholline in the presynaptic cholinergic nerve terminals through the decomposition of SNAP-25 (synaptosome-associated protein of 25,000 daltons), an integral protein for a successful coupling and the release of acetylcholline from the vesicles located within the nerve terminals 58.
After the injection, there is a high affinity fast initial crosslinking of the toxin to the specific receptors of the cell surface. This is followed by the transference of the toxin through the plasma membrane by receptor-mediated endocytosis. Finally, the toxin is released in the cytosol. This last process is accompanied by progressive inhibition of the release of acetylcholine; the clinical signs usually appear within 2-3 days, with a maximum effect within a term of 5-6 weeks after the injection.
The recovery after the intramuscular injection generally takes place within 12 weeks of the injection as new nerve terminals develop and enable the reconection of the neuron with the terminal plates. After the intradermal injection, directed to the eccrine sweat glands, the effect lasted an average of 7.5 months after the first injection in patients treated with 50 units per axilla. However, in 27,5 % of the patients, the effect duration was a year or longer 60. The recovery of the sympathetic cholinergic nerve terminals which inervate the sweat glands after the intradermal injection with BOTOX® has not been studied.
Blepharospasm / hemifacial spasm:
An additional randomized, multicentric, double blind study of parallel group which compared the safety and efficiency of 2 BOTOX® formulations (active substance derived from different Banks of Stem Cells, one of which is the current formulation of BOTOX®) in patients with mild essential blepharospasm. The study included patients with previous exposure to BOTOX® and the treatment consisted in only one course of injections in the orbicular muscle of both eyes. The dose (maximum allowed was 100 U) and the injection sites were determined by the researcher based on the reaction of the patients to the previous exposure to BOTOX®. The patients were observed during a period of 12 weeks after the treatment.
98 patients were admitted in the study (48 in the group with the current formulation of BOTOX®). The success rate of the treatment was measured by using a rating scale of five points from 0 to 4, where 0 was equal to “none” and 4 was equal to “severe” spasm incapacitating the eyelids and possibly other facial muscles. A decrease to a value of ≤ 2 in both eyes was considered a success in the treatment. The primary time point was on week 4. The average dose received by the patients was 33 U per eye, injected in 3 to 15 sites. Comparable to the previous formulation of BOTOX®, the success rate of the treatment with the current formulation of BOTOX® was approximately 90% on week 4.
In an open study, 56 patients with hemifacial spasm were injected with an initial dose of 10 to 50 units followed by 22 weeks of observation. Patients who do not show any improvement on week 4 were reinjected (5 to 50 units). The individuals were observed on weeks 2, 4 (when they require re-treatment), 6, 14 and 22. The clinical effects were assessed in the different superior and inferior facial muscles in each visit.
The 56 patients showed improvements and the 62,5% (35/56) showed a notable improvement. When assessing the superior facial muscles, an improvement was observed in all the patients. When the inferior facial muscles were assessed, all the patients were considered as responders, except 2 of them.
Strabismus
Six hundred and seventy seven adult patients with strabismus, treated with one or several injections of BOTOX® were evaluated in a retrospective case review on a large scale. 55% of these patients improved to an alignment of 10 diopters or less when they were evaluated after six months or more after the injection.
Cervical dystonia:
A randomized, multicentric, double blind, placebo-controlled study was carried out in patients with cervical dystonia (DC). This study admitted adult patients with DC and records of positive answer to the treatment with BOTOX. The study was divided in 2 treatment periods, a period in open mode during which all the patients received treatment with BOTOX®, with doses and injection sites individually by the researcher and a double blind period during which the patients considered as responders to the treatment in the open period received BOTOX® or else placebo in double blind mode. There was an interval of up to 6 weeks between these 2 periods. In general, up to 360 U of BOTOX® were administered to the patients.
214 were assessed in the open period, from which 170 were randomized to the double blind part of the study (88 in the group of BOTOX®, 82 in the placebo group). Then, a follow-up of the patients in intervals of 2 weeks up to 10 weeks after treatment was carried out. The primary variables of efficiency were a change in the position of the head (measured by the Cervical Dystonia Severity Study) and the percentage of patients showing an improvement in their condition of DC measured with the Global Physical Appraisal Scale to 6 weeks after treatment. The pain, which is an important symptom of the DC, was assessed by the patients regarding its frequency and intensity. Besides, the physician and the patient valued the functional disability.
BOTOX was clinically and statistically superior to the placebo regarding both primary efficiency variables to the 6 weeks. BOTOX® was also better statistically in a significant degree than the placebo in the reduction of the intensity and frequency of the pain. Significant improvements were also observed in the assessment of physician and patient of the functional disability on week 6.
An additional randomized, crossed, double blind study evaluated the safety and efficiency of 2 BOTOX® formulations (active substance derived from different Banks of Stem Cells, one of which is the current formulation of BOTOX®) in patients with cervical dystonia. The study admitted patients that had been previously treated with satisfactory results and the treatment of the study consisted in 2 injections, separated by an elimination period of 8 to 16 weeks. A follow-up of the patients of up to 8 to 16 weeks after each treatment with the primary measuring point of week 6 after each treatment.
135 patients were included in this study. The primary efficacy variable was the total value of severity in the Toronto Western Spasmodic Torticollis Rating Scale (TWSTRS). The secondary variables of efficiency were the value of disability for pain (from the modified TWSTRS scale) and the global appraisal of the patient and the physician on cervical dystonia. Results confirmed that the maximum clinical improvement after the BOTOX® injection happened in week 6. The average decrease in the total value in the TWSTRS scale represented an improvement of 35% regarding the basal values, and the higher average decrease in the value of disability for pain represented a decrease of 50% regarding the basal point in this point of time. The global appraisal, both from phyisicians and patients, also showed the positive effect of the treatment, for the current formulation of BOTOX®, with more than 85% and 80%, reporting success in the treatment in week 6.
Child cerebral palsy:
A multicentric, double blind, placebo-controlled was carried out in 145 children with cerebral palsy who showed spasticity of the inferior member(s), associated with dynamic muscular contracture and a pes equinus valgus position of the foot when walking. The patients were randomized to receive BOTOX® (N=72) or placebo (N=73). The total dose administered to each individual in their gastrocnemius muscles was 4 U/kg, until a maximum of 200 U during a period of 30 days. The patients were observed during a period of 12 weeks after the treatment. The primary efficiency variable, the success rate of treatment, was defined as an increase of the value ≥ 2 regarding the basal in the rating of the standard of dynamic march (when walking). The success rate was significantly higher in the BOTOX® group than in the placebo group in all the follow-up visits..
Local spasticity of upper limbs associated with cerebrovascular accident:
A double blind, multicentric, placebo-controlled study to evaluate the safety and efficiency of BOTOX in patients with spasticity in upper limbs after a cerebrovascular accident. The individuals were adults who had not received BOTOX® with 6 weeks after the cerebrovascular accident, with expanded Ashworth values 3 in the wrist and 2 in the elbow. Ninety one individuals were randomized to one of the four treatment groups (BOTOX® 90, 180 or 360 U or placebo). The primary measuring point, spasticity of the wrist flexor, was measured with the Expanded Ashworth Scale (EAS) in week 6. The secondary variables of efficiency included spasticity of elbows and finger flexors measured by EAS and Global Assessment from the Physician.
91 patients were admitted in this study (65 patients received BOTOX® at 90, 180 or 360 units). The average decrease in EAS for the wrist flexors was higher than with placebo for all the groups of BOTOX® until week 12. All the groups of treatment of BOTOX® were more effective than the placebo in the reduction of the tone of the wrist flexor measured by EAS. The elbow flexors showed similar improvement standards. Although some general variability was shown in the finger flexors (significant improvements only in weeks 1 and 3), the data reflect similar dose-response relationships. The Global Assessment of Physicians also showed a significant benefit of the dose of 180 units and 360 units.
Another double blind, multicentric, placebo-controlled study in patients with secondary spasticity of wrist and fingers to cerebrovascular accident. The study duration was 12 weeks. The primary measuring point was the spasticity of the wrist flexor in week 6, measured pursuant the Ashworth Scale (AS). Other variables included Global Assessment of the Physician, Global Assessment of the of the Patient Keeper, Scale of Disability Assessment and Principle of Objectives of Therapeutic Intervention.
126 patients were randomized to receive BOTOX® (N=64) or placebo (N=62). The injected dose of BOTOX® was between 200 and 240 U divided between the affected fingers and wrist muscles.
It was determined that BOTOX® improved the value for wrist and fingers in all the post-injection visits. In week 6, 88% of the individuals improved in the AS value with BOTOX® compared to 34% of the placebo group. All the remaining variables also showed a significant benefit compared to the placebo in week 6.
Spasmodic dysphonia
In the bigger study that has been reported, 639 patients with spasmodic dysphony of the adductor and 108 patients with spasmodic dysphony of the abductor were injected with an average dose of 3.1 3.1 units and 2.16 1.07 units of BOTOX®, respectively. The patients registered the responses in a journal, including their percentage of normal function, in a global visual analogous scale in which 100% was a normal voice and 0% was inability to emit voice. For spasmodic dysphony of the aductor, the average start of the effect was 2.4 4.3 days with a maximum average effect of 9 12.7 days. The average duration of the benefit was 15.1 12.3 weeks, the percentage of normal function increased from 52.4% 22% to 89.71% 13%. For spasmodic dysphony of the abductor, the average start of the effect was 4.1 5.5 days with a maximum average effect of 10 12.5 days. The average duration of the benefit was 10.5 12.2 weeks. The percentage of normal function increased from 54.8% 21.9% to 66.7% 23.4%, respectively.
In another series on a large scale of 169 patients (spasmodic dysphony of the adductor 88,8%, spasmodic dysphony of the abductor 1,8% and mixed spasmodic dysphony 4,1%) the median of the rating from the treatment result was excellent in 63,9% of the patients, very good in 18%, satisfactory in 14,5% and unsatisfactory in 3,5%, according to what is determined with a scale of subjective self-assessment which the patients registered in a journal. The speech speed increased after the therapy with BOTOX® approximately in 12 syllables per minute. By means of a nasendoscopy, before and after the treatment in many patients, the weakening of the vocal cords and the abolition of the spasms were confirmed.
Primary axillar hyperhydrosis:
A clinical, double blind, multicentric study was carried out in patients who showed persistent bilateral primary axillar hyperhydrosis, defined as a basal gravimetric measurement at least of 50 mg of spontaneous production of sweat in each axilla during 5 minutes at room temperature, at rest. Three hundred and twenty patients were randomized to receive 50 Units of BOTOX® (N=242) or else placebo (N=78). The responders to the treatment defined themselves as individuals showing at least a decrease of 50% regarding the basal value in axillar sweat. In the primary measuring point, in week 4 post-injection, the response rate in the BOTOX® group was 93.8% compared to 35.9% from placebo group (p1/100, 1/1,000, 1/10,000,
