Canine atopic dermatitis and the
role of dietary polyunsaturated fatty
acids and antioxidants
Canine atopic dermatitis
Atopic dermatitis is a genetically predisposed allergic skin disease that is caused by an (immunological) hypersensitivity reaction to environmental allergens such as house dust mites, pollen, mould spores etc. The allergens are inhaled, ingested or they may cross the skin barrier (Olivry, 2004).
Atopic dermatitis in dogs is characterised by inflammation of the skin and, most often, a pronounced itching.
It is estimated that 10% to 15% of dogs are affected by atopic dermatitis. This makes atopic dermatitis one of the most frequent conditions seen in small animal veterinary practice.
Pathophysiology of atopic dermatitis
Even if the exact pathogenesis of atopic dermatitis is still unclear, it is known that IgE mediated mast cell degranulation
occurs (Figure 1). IgE antibodies on dermal mast cells will bind to the allergens and activate the release of histamine, proteases, chemokines, cytokines and other mediators.
Recent investigations also suggest the importance of an epidermal barrier defect in the pathogenesis of atopic dermatitis. This may facilitate contact of environmental allergens with epidermal immune cells. Epidermal Langerhans cells with allergen specific IgE capture allergens and migrate to the regional lymph nodes. Keratinocytes are activated and secrete inflammatory cytokines and chemokines (Olivry, 2004).
The cascade of inflammatory mediators causes the response with swollen, reddened and itchy skin seen in atopic dermatitis.

Failure to down regulate the pro-inflammatory mechanisms lead to self-perpetuating inflammation and atopic dermatitis commonly develops into a chronic phase. The itching resulting from the acute phase inflammation leads to self trauma and the release of neurotransmitters, both of which may cause further tissue damage and exacerbate inflammation in a vicious circle of cause and effect. Furthermore, microbial overgrowth may also contribute to the chronic inflammation.
Fatty acids and their role in canine atopic dermatitis
Dietary polyunsaturated fatty acids (PUFAs) are known to have the capacity of modulating the inflammatory response in immune mediated inflammatory conditions like e.g. atopic dermatitis. Vertebrates can not synthesise these molecules but have to obtain them through their food.

What are polyunsaturated fatty acids?
PUFAs are fatty acids with two or more double bonds in the molecule. The PUFAs are grouped on the basis of where the last double bond is situated in the molecule. Two groups are of primary nutritional interest: The omega-3 fatty acids and the omega-6 fatty acids.
Within the omega-3 group the most important PUFAs are alfa-linolenic acid (ALA) which is found i.e. in rapeseed oil and flax oil, and eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA) both of which are primarily found in marine oils (Figure 2).
Within the omega-6 group the most important PUFAs are linoleic acid (LA) found in many vegetable oils, gamma-linolenic acid (GLA) found in evening primrose oil and borage oil and arachidonic acid (AA) mostly found in meat (Figure 2).

Modification of the inflammatory response
All cell membrane phospholipids contain omega-6 and omega-3 PUFAs. The diet composition determines the membrane PUFA composition. Upon cell death the PUFAs are released from the cell membranes by the action of phospholipases. When they are released they form the substrate for formation of eicosanoids (biological mediators) involved in the inflammatory cascade. In this respect different PUFAs give rise to different eicosanoids of which some are pro-inflammatory and others are less- or even anti-inflammatory. Both omega-3 and omega-6 groups of fatty acids have been shown to be able to reduce the clinical signs of atopic dermatitis. However, the mode of action in some aspects seems to be different for the two groups (Figure 3).

Omega-3 fatty acids
Consumption of the long-chain omega-3 fatty acids results in partial replacement of arachidonic acid (AA) in cell membranes. This changes the amount and alters the balance of eicosanoids produced. The long-chain omega-3 fatty acids EPA and DHA compete with AA for the cyclooxygenase and lipoxygenase enzymes thereby reducing production of the pro-inflammatory prostaglandin PGE2 and leukotriene LTB4 for the benefit of the less-inflammatory PGE3 and LTB5 (Figure 4).

Furthermore the long-chain omega-3 fatty acids have a number of other anti-inflammatory effects that might occur downstream of altered eicosanoid production or are independent of this. For example, the consumption of fish oil inhibits production of the pro-inflammatory cytokines IL-1, IL-6 and TNF- (Calder 1998, Endres S et al 1989), neutrophil chemotaxis, MHC II expression and antigen presentation (Calder 1998, Fujikawa M et al 1992).
Omega-6 fatty acids
On the other hand the omega-6 fatty acid GLA is a precursor for di-homo-gamma-linolenic acid DGLA. This fatty acid is a substrate for the production of PGE1, an eicosanoid with known anti-inflammatory and immunoregulatory properties. PGE1 inhibits the release of histamine – a major mediator of canine atopic dermatitis (Gueck et al, 2004) (Figure 5). Another possible mode of action of these omega-6 fatty acids has been suggested by Johnson et al (1997) who found that GLA strongly inhibits the biosynthesis of the pro-inflammatory LTB4. This inhibition is believed to be the result of a direct inhibition of the 5-lipoxygenase enzyme. Furthermore, supplementation with GLA leads to a significant increase in release of DGLA from stimulated

neutrophils whereas release of AA remains constant. A recent study has demonstrated a significant increase in LTB4 values of atopic dogs compared to normal dogs, and the results obtained suggest that LTB4 contributes significantly towards the pathogenesis of AD in dogs (Breathnach R et al. 2006). Also DGLA has been found to inhibit production of TNF- independently of cyclooxygenase activity (Dooper et al, 2003), which may even point to a third mode of action of this particular fatty acid of the omega-6 series.
The effects of the omega-3 fatty acids EPA and DHA and the omega-6 fatty acid GLA in AD thus seem to have different biochemical basis and consequently it is not likely that the effect of either group can be replaced by feeding more of the other group. On the contrary it seems reasonable to suggest that the anti-inflammatory and immunoregulatory effects of these fatty acids are additive and in some aspects maybe even synergistic.
A recent in-vitro study using mononuclear cells isolated from atopic dogs evaluated the influence of different PUFAs, alone or in combination, on immune parameters related to atopic dermatitis. The results of this study show that a combination of GLA/EPA/DHA is more effective than the individual PUFAs in reducing the proliferation of allergen challenged T-cells (Stehle, In Press).
Why are GLA and EPA/DHA better than LA and ALA?
ALA can be transformed into EPA/DHA and LA can be transformed into GLA by different elongation- and desaturation steps. The enzymes controlling these steps are present in all mammals including dogs, however in varying amounts depending on species, age, nutrition, and health condition. The rate limiting enzyme in this respect is delta-6-desaturase (Figure 6). This enzyme is not present in skin (Brosche, 2000), and the bulk of scientific research favours the view that the overall conversion of LA to GLA is reduced in patients with AD (Horrobin 2000).

As delta-6-desaturase is a rate limiting step, feeding large amounts of LA will competitively inhibit the conversion of ALA to EPA/DHA and visa versa.
So in order to have sufficient quantities as well as an adequate balance of GLA and EPA/DHA, these fatty acids must be fed directly rather than their precursors LA and ALA thereby bypassing the rate-limiting delta-6-desaturase enzymatic step.
Documentation of the efficacy of PUFA supplementation in atopic dermatitis
The effect of PUFAs on the inflammatory response in canine atopic dermatitis has been widely investigated. In most of these studies the effect has been good, but either the studies have not been blinded and placebo-controlled, or the PUFA content of the normal diet has not been taken into consideration, putting the study results into question. In 2004 indisputable proof of the effect was delivered by Sævik et al. (Figure 7)
who conducted a randomised, double-blinded, placebo-controlled multi-centre clinical trial of 12 weeks duration with 60 dogs with atopic dermatitis. The purpose was to evaluate the steroid sparing effect of essential fatty acid supplementation

The daily intake from the feed was comparable to the daily intake of a typical dry food, except from GLA, which was approximately 3.5 times higher than in the typical dry food, which strengthened the total amount of GLA even further. The results of the study showed that the essential fatty acid supplementation has a significant corticoid sparing effect in dogs with atopic dermatitis.
PUFA supplementation improves the skin barrier and ameliorates dry skin and coat
The skin functions is the primary defence to the external environment. The critical barrier function is mediated by the outmost layer of the epidermis, the stratum corneum. The barrier function is maintained by epidermal lipids, especially those interspersed between the layers of stratum corneum.
The alteration of the skin lipids can favour the development of an allergic reaction.
Inman et al (2001) found that the deposition of lipid in the stratum corneum of atopic canine skin was markedly heterogeneous compared to that of normal canine skin (Figure 8). Many areas within the AD stratum corneum were devoid of lipid. When present, the lamellae often exhibited an abnormal and/or incomplete structure. The values for both continuity and thickness of intercellular lipid were significantly less in the non-lesional atopic than in the normal canine skin (figure 9A B). This means that the function of the epidermal barrier is impaired in AD patients, even if there are no distinct clinical signs. This abnormal

epidermal barrier function is reflected in an increased transepidermal water loss, and results in dry skin and a dull and lifeless coat.
Linoleic acid (LA) has been found to play a significant role in controlling water permeability in skin (Hansen and Jensen 1985). An explanation for this effect is suggested by Sinclair et al 2002, who found that LA is incorporated into skin ceramides – a group of chemical derivatives of fatty acids with an important role in limiting loss of water and solutes through the skin and at the same time preventing the ingress of harmful substances.
The role of antioxidants in atopic dermatitis
Free radical formation and elimination
Inflammatory cell activation and dysregulated cytokine production appear to play a critical role in the pathogenesis of atopic dermatitis. In this setting, alterations in the flux of reactive oxygen species (ROS) are believed to have profound influences on the pathophysiology (Okayama 2005, Tsukahara 2003).
The reactive radicals and oxidants released from mast cells and eosinophils may injure cells and tissue directly via oxidative degradation of essential cellular components as well as injure cells indirectly by altering the normal enzymatic balance. Free radicals may also initiate and/or amplify inflammation via the upregulation of several dif-ferent genes involved in the inflammatory response,

such as those that code for proinflammatory cytokines and adhesion molecules (Conner 1996). Free radicals are involved in regulation of prostaglandin H synthase, the rate limiting enzyme for prostaglandin production.
A large number of factors are known to modulate the amount of free radical formation in the body. Most important in this respect are the antioxidants. Antioxidants stabilise biological membranes and help prevent widespread cellular destruction by willingly donating components to stabilise free radicals (figure 10). More important, antioxidants return to the surface of the cell to stabilise rather than damage other cellular components. Consequently it is logical that antioxidant status is an important factor for the manifestation of atopic dermatitis (Wolber, 1996).
The physiological antioxidants are not independently capable of detoxifying the free radicals released in connection with AD. An exogenous supply of dietary antioxidants is thus of major importance in slowing down the progression of inflammation. An exogenous supply of antioxidants would be especially indicated in dogs that are fed essential fatty acid supplements as the highly unsaturated fatty acids are exposed to physiological oxidation, which would intensify the oxidative stress (Fang 2002).

Why are small amounts of several antioxidants better than a large amount of only one antioxidant?
Epidemiological studies have consistently shown that a diet with lots of fruit and vegetables rich in dietary antioxidants may prevent chronic diseases, particularly cancer and cardiovascular disease. However, when the role of individual antioxidants is examined by epidemiological studies or supplementation trials the results are not as clear cut as those with fruit and vegetables, and are often quite disappointing (Slattery et al 1997). Several theories have been proposed as explanations for this. One such theory suggested by Eastwood (1999) is that different antioxidants due to different solubilities under varying pH, ionic strength, electrical charge etc. may be active in different types of cells or anatomical compartments. This suggests that there would be synergism between different antioxidants and it raises the prospect that a variety of antioxidants are necessary for optimal efficacy. Whether this theory will prove to be correct, or not, the prevailing scientific opinion today is that small amounts of a large number of antioxidants should be preferred over a large amount of only one antioxidant.

Atopisk dermatit hos hund
Atopisk dermatit är en genetiskt predisponerad allergisk hudsjukdom, som orsakas av en (immunologisk) överkänslighetsreaktion
mot allergener i miljön, såsom dammkvalster i bostaden, pollen, mögelsporer etc. Allergenerna inandas, sväljs eller kan passera hudbarriären (Olivry, 2004).
Atopisk dermatit hos hund karakteriseras av hudinflammation och, oftast, uttalad klåda.
Det uppskattas att 10 % till 15 % av hundarna drabbas av atopisk dermatit. Detta gör atopisk dermatit till ett av de vanligaste besvären som ses på små veterinärkliniker.
Patofysiologi vid atopisk dermatit
Även om den exakta patogenesen för atopisk dermatit fortfarande är oklar, känner vi till att IgE-medierad mastcellsdegranulation förekommer (Figur 1). IgE-antikroppar på mastceller i huden binds till allergener och aktiverar frisättningen av histamin, proteaser, kemokiner, cytokiner och andra mediatorer.
Nya undersökningar tyder också på betydelsen av en epidermal barriärdefekt i patogenesen för atopisk dermatit. Denna kan eventuellt underlätta kontakten mellan allergener i miljön och epidermala immunceller. Epidermala Langerhansceller med allergenspecifikt IgE fångar upp allergener och vandrar till de lokala lymfkörtlarna. Keratinocyter aktiveras och utsöndrar inflammatoriska cytokiner och kemokiner (Olivry, 2004).
Kaskaden av inflammatoriska mediatorer orsakar den reaktion med svullen, rodnad och kliande hud som ses vid atopisk dermatit.

Misslyckad nedreglering av de proinflammatoriska mekanismerna leder till självunderhållande inflammation och atopisk dermatit utvecklas vanligtvis till en kronisk fas. Klådan, som är ett resultat av den akuta fasens inflammation, leder till självtrauma och frisättning av neurotransmittorer, vilka båda kan orsaka ytterligare vävnadsskada och förvärra inflammationen i en ond cirkel av orsak och verkan. Dessutom kan även en alltför hastig tillväxt av mikroorganismer bidra till den kroniska inflammationen.
Fettsyror och dess funktion vid atopisk dermatit hos hund
Vi vet att fleromättade fettsyror (polyunsaturated fatty acids, PUFA) i kosten har förmågan att modulera den inflammatoriska reaktionen vid immunmedierade inflammatoriska tillstånd, som t.ex. atopisk dermatit. Vertebrater kan inte syntetisera dessa molekyler, utan måste erhålla dem genom födan.

Vad är fleromättade fettsyror?
PUFA är fettsyror med två eller fler dubbelbindningar i molekylen. PUFA grupperas utifrån var den sista dubbelbindningen i molekylen är placerad. Två grupper är av störst intresse näringsmässigt: omega-3-fettsyror och omega-6-fettsyror.
Inom omega-3-gruppen är de viktigaste PUFA alfalinolensyra (ALA), som återfinns i rapsfröolja och linolja, samt eikosapentaensyra (EPA) och dokosahexaensyra (DHA), vilka båda återfinns i marina oljor (Figur 2).
Inom omega-6-gruppen är de viktigaste PUFA linolsyra (LA), som återfinns i många vegetabila oljor, gammalinolensyra (GLA), som finns i nattljusolja och gurkörtolja, samt arakidonsyra (AA), som huvudsakligen återfinns i kött (Figur 2).

Modifiering av den inflammatoriska reaktionen
Alla fosfolipider i cellmembranet innehåller de fleromättade fettsyrorna omega-6 och omega-3. Kostsammansättningen bestämmer membranets sammansättning av PUFA. Vid celldöd frisläpps PUFA från cellmembranen under inverkan av fosfolipaser. När dessa frisläpps, skapar de substratet för bildningen av eikosanoider (biologiska mediatorer), som är inblandade i den inflammatoriska kaskaden. I detta avseende ger olika PUFA upphov till olika eikosanoider, av vilka vissa är proinflammatoriska och andra är mindre inflammatoriska eller till och med antiinflammatoriska. Fettsyregrupperna omega-3 och omega-6 har båda visat sig ha förmåga att reducera de kliniska tecknen på atopisk dermatit. Emellertid tycks verkningssättet ur vissa aspekter skilja sig mellan de två grupperna (Figur 3).

Omega-3-fettsyror
Förtäring av de långkedjiga omega-3-fettsyrorna resulterar i en partiell ersättning av arakidonsyra (AA) i cellmembranen. Detta förändrar mängden av och ändrar balansen hos de eikosanoider som produceras. De långkedjiga omega-3-fettsyrorna EPA och DHA konkurrerar med AA om enzymerna cyklooxygenas och lipooxygenas och reducerar därigenom produktionen av det proinflammatoriska prostaglandinet PGE2 och leukotrien LTB4 till förmån för de mindre inflammatoriska PGE3 och LTB5 (Figur 4).

Dessutom har de långkedjiga omega-3-fettsyrorna ett antal andra antiinflammatoriska effekter, som kan förekomma nedströms om den förändrade eikosanoidproduktionen eller är oberoende av denna. Förtäring av fiskolja hämmar till exempel produktionen av de proinflammatoriska cytokinerna IL-1, IL-6 och TNF-α (Calder 1998, Endres S et al 1989), neutrofil kemotaxis, uttryck av MHC II och presentation av antigen (Calder 1998, Fujikawa M et al 1992).
Omega-6-fettsyror
Å andra sidan är omega-6-fettsyran GLA en prekursor till dihomogamma linolensyra DGLA. Denna fettsyra är ett substrat för produktionen av PGE1, en eikosanoid med kända antiinflammatoriska och immunreglerande egenskaper. PGE1 hämmar frisättningen av histamin – en viktig mediator till atopisk dermatit hos hund (Gueck et al, 2004) (Figur 5). Ett annat möjligt verkningssätt för dessa omega-6-fettsyror har föreslagits av Johnson och medarbetare (1997), som upptäckte att GLA kraftigt hämmar biosyntesen av det proinflammatoriska LTB4. Denna hämning tros vara resultatet av en direkt hämning av enzymet 5-lipoxygenas. Dessutom leder tillskott med GLA till en markant ökning av frisättningen av DGLA från stimulerade

neutrofiler, medan frisättningen av AA förblir konstant. En ny studie har påvisat en betydande ökning av LTB4-värden hos atopiska hundar i jämförelse med normala hundar, och de uppnådda resultaten tyder på att LTB4 på ett markant sätt bidrar till patogensen vid AD hos hund (Breatnach R et al, 2006). Även DGLA har visats hämma produktionen av TNF – oberoende av cyklooxygenas-aktivitet (Dooper et al, 2003), vilket också kan peka på ett tredje verkningssätt hos denna särskilda fettsyra i omega-6-serien.
Effekten av omega-3-fettsyrorna EPA och DHA och omega-6-fettsyran GLA vid AD verkar följaktligen ha olika biokemiska utgångspunkter och det är därför inte troligt att effekten av någondera gruppen kan ersättas genom att utfodra mer av den andra gruppen. Tvärtom verkar det rimligt att föreslå att dessa fettsyrors antiinflammatoriska och immunreglerande effekter är additiva och ur vissa aspekter kanske även synergistiska.
En ny in-vitrostudie, som använde sig av mononukleära celler som isolerats från atopiska hundar, utvärderade påverkan av olika PUFA, en och en eller i kombination, på immunparametrar i relation till atopisk dermatit. Resultaten av denna studie visar att en kombination av GLA/EPA/DHA är mer effektiv än de enskilda PUFA för att reducera proliferationen av T-celler som utmanats av allergen (Stehle, In Press).
Varför är GLA och EPA/DHA bättre än LA och ALA?
ALA kan omvandlas till EPA/DHA och LA kan omvandlas till GLA genom olika förlängnings- och desaturationssteg. De enzymer som kontrollerar dessa steg förekommer hos alla däggdjur, däribland hundar, dock i varierande mängder beroende på art, ålder, näringstillförsel och hälsotillstånd. Det hastighetsbegränsande enzymet i detta avseende är delta-6-desaturas (Figur 6). Detta enzym förekommer inte i huden (Brosche, 2000), och huvuddelen av den vetenskapliga forskningen ansluter sig till uppfattningen att den totala omvandlingen av LA till GLA är reducerad hos patienter med AD (Horrobin, 2000).

Eftersom delta-6-desaturas är ett hastighetsbegränsande steg, kommer utfodring av stora mängder LA att på ett konkurrerande sätt hämma omvandlingen av ALA till EPA/DHA och vice versa.
För att ha tillräckliga kvantiteter såväl som en adekvat balans av GLA och EPA/DHA, måste dessa fettsyror sålunda utfodras direkt, i stället för deras prekursorer LA och ALA, för att därigenom kringgå det hastighetsbegränsande steget av enzymet delta-6-desaturas.
Dokumentation av effektiviteten av tillskott av PUFA vid atopisk dermatit
Effekten av PUFA på den inflammatoriska reaktionen vid atopisk dermatit hos hund har undersökts i stor utsträckning. I de flesta av dessa studier har effekten varit god, men antingen har de inte varit placebokontrollerade blindstudier eller har innehållet av PUFA i den normala kosten inte beaktats, vilket har gjort att studieresultaten har ifrågasatts. 2004 uppvisades obestridliga bevis på denna effekt av Sævik och medarbetare (Figur 7), som ledde en randomiserad, placebokontrollerad klinisk multicenterstudie som pågick i 12 veckor med 60 hundar med atopisk dermatit. Syftet var att utvärdera den steroidsparande effekten av ett tillskott av essentiella fettsyror (Xxx).

Det dagliga intaget från födan var jämförbart med det dagliga intaget av ett typiskt torrfoder, förutom från GLA, som var ungefär 3,5 gånger högre än i det typiska torrfodret, vilket förstärkte den totala mängden av GLA ännu mer. Resultaten av studien visade att tillskottet av essentiella fettsyror har en markant kortisonsparande effekt hos hundar med atopisk dermatit.
Tillskott av PUFA förbättrar hudbarriären och förbättrar torr hud och päls
Hudens funktioner är det primära försvaret mot den externa miljön. Den livsviktiga barriärfunktionen förmedlas av det yttersta lagret av epidermis, stratum corneum. Barriärfunktionen upprätthålls genom epidermala lipider, särskilt de som är utspridda mellan lagren av stratum corneum.
Förändringen av hudens lipider kan understödja utvecklingen av en allergisk reaktion.
Inman och medarbetare (2001) upptäckte att avlagringen av lipider i stratum corneum hos hundar med atopisk hud var påtagligt heterogen i jämförelse med den i normal hundhud (Figur 8). Många områden inom stratum corneum vid AD var tomma på lipider. När de förekom, uppvisade lamellerna ofta en avvikande och/eller ofullständig struktur. epidermala barriärfunktion avspeglas i en ökad transepidermal vattenförlust och resulterar i torr hud och en matt och livlös päls. Linolsyra (LA) har visat sig spela en betydelsefull roll i kontrollen av hudens vattenpermeabilitet (Hansen och Jensen, 1985).
En förklaring till denna effekt föreslås av Sinclair och medarbetare 2002, som konstaterade att LA är införlivat i hudens ceramider – en grupp av kemiska derivat av fettsyror med den viktiga funktionen att begränsa förlusten av vatten och lösningar genom huden och samtidigt förhindra inträngande av skadliga substanser.
Antioxidanters funktion vid atopisk dermatit
Bildning och elimination av fria radikaler
Inflammatorisk cellaktivering och dysreglerad cytokinproduktion förefaller spela en avgörande roll i patogenesen för atopisk dermatit. I denna omgivning tros förändringar i flödet av reaktiva syreradikaler (reactive oxygen species, ROS) ha genomgripande påverkan på patofysiologin (Okayama 2005, Tsukahara 2003).
De reaktiva radikalerna och oxidanterna som frisätts från mastceller och eosinofila leukocyter kan skada celler och vävnad såväl direkt via oxidativ nedbrytning av viktiga cellkomponenter som indirekt genom att förändra den normala enzymbalansen. Fria radikaler kan också initiera och/eller utveckla inflammation via en uppreglering av flera olika gener som är inblandade i den inflammatoriska reaktionen,

såsom de som kodar för proinflammatoriska cytokiner och adhesionsmolekyler (Conner, 1996). Fria radikaler är inblandade i regleringen av prostaglandin H-syntas, det hastighetsbegränsande enzymet för prostaglandinproduktion.
Vi vet att ett stort antal faktorer modulerar den mängd fria radikaler som bildas i kroppen. Viktigast i detta avseende är antioxidanterna. Antioxidanter stabiliserar biologiska membraner och hjälper till att förhindra omfattande cellförstörelse genom att beredvilligt donera komponenter för att stabilisera de fria radikalerna (figur 10). Av större vikt är att antioxidanterna återvänder till cellytan för att stabilisera snarare än skada andra cellkomponenter. Därför är det logiskt att antioxidantstatus är en viktig faktor för manifestationen av atopisk dermatit (Wolber, 1996).
De fysiologiska antioxidanterna är inte på egen hand kapabla att avgifta de fria radikaler som frisätts i samband med AD. Exogen tillförsel av antioxidanter i kosten är därför av stor vikt för att minska progressionen av inflammation. Exogen tillförsel av antioxidanter skulle vara särskilt indicerat till hundar som utfodras med tillskott av essentiella fettsyror, eftersom de starkt omättade fettsyrorna exponeras för fysiologisk oxidation, vilket skulle förstärka den oxidativa påfrestningen (Fang, 2002).

Varför är små mängder av flera antioxidanter bättre än en stor mängd av enbart en antioxidant?
Epidemiologiska studier har genomgående visat att en kost med mycket frukt och grönsaker naturligt rik på antioxidanter kan förebygga kroniska sjukdomar, särskilt cancer och kardiovaskulär sjukdom. När de enskilda antioxidanternas funktion undersöks genom epidemiologiska studier eller försök med tillskott, är resultaten dock inte så entydiga som för dem med frukt och grönsaker, och är ganska ofta en besvikelse (Slattery et al, 1997). Flera teorier har framlagts som förklaring till detta. En sådan teori, föreslagen av Eastwood (1999), är att olika antioxidanter beroende på olika löslighet under varierande pH, jonstyrka, elektrisk laddning etc. kan vara aktiva i olika celltyper eller anatomiska rum. Detta tyder på att det skulle vara synergism mellan olika antioxidanter och det tar upp möjligheten att en mängd olika antioxidanter är nödvändiga för optimal effektivitet. Oavsett om denna teori kommer att visa sig vara korrekt eller inte, är den rådande vetenskapliga uppfattningen i dag att små mängder av ett stort antal antioxidanter bör föredras framför en stor mängd av enbart en antioxidant.

Anesthesiologists to have new tool that works within minutes, upon European approval
KENILWORTH, N.J., June 2, 2008 – Schering-Plough Corporation (NYSE: SGP) today reported that the Committee for Medicinal Products for Human Use (CHMP) of the European Medicines Agency (EMEA) has recommended approval of xxx for routine reversal of the muscle relaxants xxx or xxx and for immediate reversal of xxx in adults, and for routine reversal following xxx in children and adolescents (2-17 years of age). xxx and xxx are marketed in Europe under the trade names xxx and xxx, respectively.
The CHMP recommendation serves as the basis for a European Commission approval of this novel medicine. Upon European approval, xxx will be the first and only selective relaxant binding agent (SRBA) and will enable anesthesiologists to rapidly and predictably reverse both moderate and deep muscle relaxation induced by xxx and xxx. xxx creates the opportunity for anesthesiologists to maintain optimal muscle relaxation to the end of a surgical procedure and to reverse the block in minutes.
“This is an important first step in realizing the promise of our Organon BioSciences acquisition,” said Fred Hassan, chairman and CEO of Schering-Plough. “This was a relatively unknown asset before our acquisition announcement last March, and we are pleased we saw this opportunity.”
“This product has the potential to modernize the use of anesthesia around the world,” added Thomas P. Koestler, Ph.D., executive vice president and president, Schering-Plough Research Institute.
Muscle relaxants play several critical roles in general anesthesia: facilitating intubation (the insertion of a tube to assist a patient with breathing during surgery); improving surgical conditions; and reducing the risk of complications during surgery. Reversal agents are given to stop the effects of muscle relaxants, allowing patients to breathe on their own again. Current reversal agents are slow and associated with undesirable side effects. xxx will allow anesthesiologists to optimize the use of muscle relaxants.
“The data in the clinical trials demonstrate that xxx will give anesthesiologists greater control over muscle relaxation, in a way that has never been available to us before,” said Professor Rajinder Mirakhur, Professor of Anaesthetics at The Queen’s University of Belfast, Northern Ireland, a principal investigator in the xxx clinical trials program. “The drug allows us to reverse the effects of xxx quickly and at the moment that we want during a surgical procedure.”
The CHMP positive opinion is based on a comprehensive clinical trial program including the results from the SIGNAL and AURORA clinical trials.
The SIGNAL trial involved adult patients undergoing surgery. xxx was administered, at a standard dose, to allow intubation, followed by maintenance doses as required. When 1-2 post-tetanic counts (PTC) were observed following neuromuscular stimulation (deep block), patients were administered either xxx or xxx plus xxx, the current standard of care in reversal. SIGNAL clinical findings demonstrate that the median times to reversal of muscle relaxation to a train-of-four (TOF) ratio of 0.9 occurred in 2.7 minutes in the xxx group and 49.0 minutes in the xxx/xxx group.
The AURORA trial involved adult patients undergoing surgery. xxx was administered, at a standard dose, to allow intubation, followed by maintenance doses as required. At the re-appearance of the second twitch (T2) in a TOF stimulation (moderate block) patients were administered either xxx or xxx plus xxx, the current standard of care in reversal. AURORA clinical findings demonstrate that the median times to reversal of muscle relaxation to a TOF ratio of 0.9 occurred in 1.4 minutes in the xxx group, and in 17.6 minutes in the xxx/xxx group.
About xxx
xxx is a novel selective reversal binding agent (SRBA) under development by Schering-Plough Corporation. It will offer anesthesiologists the ability to reverse both moderate as well as deep muscle relaxation induced by the commonly used muscle relaxants xxx/xxx/xxx (xxx xxx) or xxx/xxx (xxx xxx), within minutes, enabling greater control over muscle relaxation during and after surgery. xxx works in an entirely novel way to encapsulate the muscle relaxant molecule and restore muscle function so that patients can breathe quickly on their own again.
In clinical trials to date, xxx has demonstrated the ability to rapidly reverse moderate and deep depths of xxx-induced muscle relaxation, thereby enabling control of the onset and offset of skeletal muscle relaxation through the use of both drugs1. xxx also has demonstrated the ability to reverse the effects of muscle relaxation induced by xxx xxx. The most common adverse reactions (incidence greater than 5 percent) seen with xxx in clinical trials were anesthetic complications generally due to restoration of muscle function (8.0 percent with xxx) and dysgeusia (impairment of the sense of taste) (12.6 percent).
Schering-Plough acquired xxx through its combination with Organon BioSciences on November 19, 2007. xxx is currently under review for marketing approval in the U.S., Europe and Japan. In March 2008, the U.S. Food and Drug Administration Advisory Committee on Anesthetics and Life Support voted unanimously to recommend xxx for approval.

U.S. FDA Clears GE’s New MR System; Simple Design Will Become New Platform of the Future
TORONTO, MAY 5, 2008 – GE Healthcare today announced it has received clearance from the U.S. Food and Drug Administration for the company’s new xxx magnetic resonance (MR) scanner, the latest addition to GE’s xxx family of MR systems. The new xxx was introduced at the International Society of Magnetic Resonance in Medicine in Toronto, May 3-9.
Equipped with the industry’s most powerful gradients, easy-to-use workflow features and the company’s advanced Thermal Management System, the xxx delivers up to 60 percent additional anatomical coverage and resolution unit per time. The system also allows for up to five times the imaging performance over previous generations, increasing the freedom for advanced application development, including: a routine liver exam in 15 minutes and a full breast exam in only two sequences.
“GE is committed to pushing the boundaries for MRI capabilities and we believe this new product will do that now and in the future,” said Jim Davis, vice president and general manager of GE Healthcare’s MR business. “We expect this product to be a robust and clinically capable MR system that will simplify MR exams without compromising quality or productivity.”
The xxx features a newly designed RF Transmit system maximizing performance with a 17 percent gain in scanning efficiency. When combined with GE’s use of high-density surface coils, the optical receive chain is a critical path for ensuring clear signal reception and data analysis. Revolutionizing imaging capabilities with new parallel imaging technique
In efforts to improve full-body imaging on xxx MR, GE Healthcare’s xxx features a newly developed parallel imaging technique entitled xxx, Auto Calibrating Reconstruction for Cartesian imaging. Den nya xxx lanserades på den årliga kongressen International Society of Magnetic Resonance in Medicine i Toronto, den 3―9 maj.
Utrustad med marknadens kraftfullaste gradienter, lättanvända arbetsflödeselement och företagets avancerade Therma Management System, ger xxx upp till 60 procent ytterligare anatomisk täckning och upplösningsenhet per tid. Systemet medger även upp till fem gånger bättre bildprestanda än tidigare generationers, vilket ger ökad frihet för avancerad tillämpningsutveckling, däribland: rutinmässig leverundersökning på 15 minuter och fullständig bröstundersökning med endast två sekvenser.

Eftersom denna teknik på ett effektivt sätt griper sig an ”aliasing”-artefakten, är xxx 2.0 nu mer robust vid utförande av små skanningundersökningar av synfält (FOV).
Förenklat användargränssnitt, patientupplevelse och automatiserad ackvisition och bildbehandling
Starkt motiverad av efterfrågan från kunderna om förbättrade kliniska möjligheter, har xxx utformats kring kliniska arbetsflödesbehov med starkt fokus på att förbättra produktiviteten. Med tanke på detta, innehåller systemet även driftseffektiviteter som det nydesignade löstagbara patientbordet och det allra första manöverbordet för rumsbruk.
xxx innehåller dessutom automatiserad ackvisition baserad på konceptet touch and go-protokoll. Systemets perspektiv tillåter större fokusering på patienten och kompatibla protokoll och bildbehandling bland driftstekniker.
Sammantaget reduceras tiden för iordningställande i skannerrummet tack vare dessa attribut med upp till 71 procent och dessutom reduceras antalet åtgärder tack vare det nyutvecklade användargränssnittet med så mycket som 68 procent.
