Source text - French L’UFH était prescrite en utilisant un protocole standard ciblant des valeurs d’activated partial thromboplastin time (APTT) situées entre 55 et 90. Le protocole permettait l’ajustement du bolus et du débit initial en fonction du poids du patient. L’information apparaissant au protocole permettait également la prescription d’un bolus standard de 5000 unités (U) et un débit initial de 18 U/kg/h pour les patients avec maladie veineuse thrombo-embolique ou 15 U/kg/h pour les patients avec maladie cardiaque ou vasculaire cérébrale. LMWH était prescite en fonction du poids du patient. Les doses prophylactiques d’UFH (sous-cutanées) et de LMWH (dose quotidienne d’enoxaparine de 40 mg ou moins ou de daltéparine 5000 unités ou moins) ont été exclues. Pour la détermination de l’impact sur les saignements, les valeurs de APTT étaient catégorisées (< 100; 100-180; >180) à partir de la valeur maximale de APTT de l’hospitalisation. Un APTT de 100 a été retenu après consultation de la littérature suggérant que cette valeur permettait le mieux de départager un statut d’anticoagulotherapie thérapeutique de supra thérapeutique (11). L’impact de la prise d’autres médicaments sur la survenue de saignements a également été étudiée avec une emphase sur les platelets active drugs, la warfarin, l’acenocoumarol, les thrombolytics, les direct thrombin inhibitors et les non-steroidal anti-inflammatory (NSAID). Les données permettaient de déterminer si ces médicaments avaient été reçus durant l’hospitalisation de référence, mais n’indiquaient pas s’ils avaient été pris de façon concomitante avec UFH ou LMWH.
Translation - English UFH was prescribed according to a standard protocol targeting levels of activated partial thromboplastin time (aPTT) between 55 and 90. The protocol also provided for adjustment of the bolus and initial infusion rate based on patient weight. The information appearing in the protocol provided for prescription of a standard bolus of 5000 units (U) and an initial infusion rate of 18 U/kg/h for the patients with venous thromboembolic disease and 15 U/kg/h for the patients with cardiac or cerebrovascular disease. LMWH was dosed according to patient weight. Prophylactic doses of UFH (subcutaneous) and LMWH (daily enoxaparine 40 mg or less, or dalteparine 5000 Units or less) have been excluded. In order to determine the impact of bleeding, the aPTT levels were categorized (< 100; 100–180; > 180) based on the maximum aPTT level during hospitalization. An aPTT of 100 was selected after examining the literature suggesting that this level better distinguished between therapeutic and supratherapeutic anticoagulant-therapy status (11). The impact on bleeding of taking other medication was also studied, with emphasis on platelet active drugs, warfarin, acenocoumarol, thrombolytics, direct thrombin inhibitors, and nonsteroidal antiinflammatory drugs (NSAIDs). The data made it possible to determine whether the medications were received during the reference hospitalization, but did not indicate whether they were taken concomitantly with UFH or LMWH.
