La defecografía es un estudio radiológico donde se examina la porción Terminal del intestino grueso, y donde el paciente reproduce el acto defecatorio. Está recomendada en el estudio de la constipación severa intratable (que no responde al tratamiento médico), en su variable de obstrucción defecatoria.
Objetivo
Comunicar los hallazgos de la defecografía en nuestro medio.

Diseño
Retrospectivo, descriptivo
Población
Adultos mayores de 14 años con constipación severa intratable.
Estudiamos pacientes con obstrucción defecatoria exclusivamente, ya que les realizamos previamente tránsito colónico descartando tránsito lento.
Método
La defecografía la realizamos con puré instantáneo agregándole como sustancia de contraste bario en polvo. Colocamos al paciente en posición sentada lateral, introducimos la sustancia y medimos los volúmenes rectales. Realizamos radioscopía y radiografias en reposo, contracción, evacuación con esfuerzo y postevacuación.
Medimos los ángulos rectoanales, descenso pelviano y defectos anatómicos.
Resultados
Realizamos 39 defecografias entre enero 2004 y diciembre 2005. Los hallazgos defecográficos fueron normales en 9, rectoceles en 12 (5 menores a 2 cm), intususcepción recto-rectal leve en 5, contracción paradojal del puborrectal en 5, descenso perineal en 4 y megarrecto en 4.
Conclusiones
La defecografía para nosotros es un método muy útil para diagnosticar tipo de obstrucción defecatoria.
Translation - English Background
Defecography is a radiological study where the last portion of the colon (big intestine) and where patients repeat the defecatory act. It is recommended in the study of the intractable severe constipation (does not respond to medical treatment), in its variable of defecatory obstruction or outlet syndrome.
Objective
To communicate defecography findings in our environment.
Place of treatment
Coloproctology Department of a Private Hospital.
Design
Retrospective, descriptive.
Population
Over 14 year-old adults with intractable severe constipation.
We studied patients with defecatory obstruction exclusively, because we previously performed colonic transit study to put aside slow transit.
Method
We performed defecography with instant mashed potatoes and powder barium as contrasting substance. We placed the patient in side sitting position, introduced the substance and measured the rectal volumes. We performed resting radioscopy and radiography, contraction, evacuation with effort and post evacuation.
We measured the recto-anal angles, the pelvic descent (lowering) and anatomical defects.
Results
From January 2004 and December 2005 we performed 39 defecographies. Defecographic findings were normal in 9 cases, rectocele in 12 cases (5 smaller than 4 cm), mild rectum-rectal intususception in 5 cases, paradoxical contraction of the puborectal in 5 cases, perineal descent (lowering) in 5 cases and megarectum in 4 cases.
Conclusions
Defecography for us is a very useful method to diagnose the kind of defecatory obstruction or outlet syndrome.
