Tabla I: Acciones del alcohol
Translation - English Diagnosis and treatment of alcohol dependence
Abstract
We describe the fundamental aspects of the diagnosis and treatment of alcohol dependence. We analyse the steps taken for a systematic diagnosis (DSM-IV and ICD-10 criteria, diagnostic questionnaires on alcohol dependence and biological markers) and the treatment for alcohol cessation.
Key words: Alcohol, dependence, diagnosis, treatment.
Throughout history, alcohol consumption has been considered to be beneficial, especially in our country where cultures and traditions revolve around wine. Epidemiological data from national health surveys conducted between 1993 and 2003 reveals that more than 60% of the Spanish population regularly consume alcohol, with a 4-7% prevalence of heavy drinkers (more than 40g/ day for men and 25 g/ day for women)¹, ². During the 1980’s there were significant changes in alcohol consumption habits. There was a shift from the daily consumption of wine by males over 20 years of age, to the consumption of beer and distilled drinks intermittently (at weekends) by adolescents of both sexes¹, ².
During the last few decades, developments have been made into the study of the damage caused by alcohol consumption. The relation between alcohol consumption and its impact depends on the quantity of alcohol intake and the pattern of consumption. The bio-chemical effects, depending on the amount of alcohol intake and the pattern of consumption, are closely linked with chronic effects at an individual level, whereas intoxication is associated with acute effects such as accidents, violence and social conflict. Alcohol dependence resulting from long-term consumption has severe consequences, both acute and chronic, and is linked to more than 60 physical and mental illnesses, as well as numerous social problems affecting those around people who drink, with violence being one of the clear repercussions. As many of these effects can be seen when only small quantities of alcohol are consumed, there are currently no safe recommended levels of intake, or that are considered to be beneficial for the health of the population as a whole³.
Aetiology and pathogenesis of alcohol-related disorders
The factors involved in the aetiology of alcoholism are genetic, psycho-social and environmental. Therefore, when carrying out research it is necessary to observe the 3 basic areas of the patient’s life: personal, family and social. The phenomena which contribute to the development of dependence are those directly linked to the reward system (positive reinforcement) and those which involve other systems whereby alcohol consumption acts as a negative reinforcement. Both processes involve different transmission systems. In alcohol dependence, this involves the anatomical reward circuits; the medial forebrain bundle, the ventral segmental area, the lateral hypothalamus and areas of the prefrontal cortex. Also involved are the neurotransmitters dopamine, noradrenalin, serotonin, glutamate, endogenous opioids, GABA and the calcium channels.
Acute alcohol intake causes sedation and euphoria by releasing dopamine, inhibiting the calcium canals, inhibiting the NMDA glutamate receptors (N-methyl-D-aspartate), and potentiating GABA. Chronic consumption causes desire by decreasing dopamine levels, increasing calcium channels, increasing the NMDA receptors, and decreasing GABA. During withdrawal, a state of neuronal excitement occurs, which involves an increase in the number of NMDA receptors and calcium channels, and a decrease in GABAergics. NMDA receptor activity in the hippocampus may cause convulsions and an increase in dopamine release in the hippocampus may cause hallucinations and delirium4.
Clinical pharmacology
Ethyl alcohol or ethanol (CH3-CH2-OH) is a clear, colourless, volatile, inflammable and very hydro soluble liquid. Its molecule is small and non-polar, which allows it to pass easily through biological membranes. One gram of alcohol supplies 7.1 kilocalories to the body. It is absorbed rapidly into the stomach (20%), and small intestine, spreading easily. The rate of absorption depends on the presence of food in the stomach, and the quantity and type of alcohol ingested. Part of the alcohol is metabolized in the stomach by the alcohol dehydrogenase (ADH), which is lower in women. It is rapidly distributed throughout the body, except for the fatty tissue, and reaches its maximum concentration level around 30-90 minutes after ingestion. It easily crosses all barriers, is predominantly hepatically metabolised and is mainly eliminated renally.
Alcohol produces considerable pharmacological effects depending on blood alcohol concentration, type of consumption (acute or chronic), and the characteristics of the individual. The duration of an episode of intoxication depends on factors such as the quantity and type of alcoholic beverage consumed the speed of ingestion, and absorption depending on whether it was consumed with or without food. When intoxication becomes progressively more intense, it goes through three stages; psychomotor excitation, in-coordination with mental depression, and coma. It may also cause amnesia in relation to the events occurring during the period of intoxication. As tolerance develops, the motor, sedative and anxiolytic effects of alcohol decrease in intensity5. The phrase ‘problematic consumption’ is used whenever alcohol related problems occur. Harmful consumption (ICD-10) refers to the existence of medical or psychological problems resulting from alcohol consumption, irrespective of the quantity consumed. Alcohol abuse (DSM-IV-TR) refers to a maladaptive pattern that leads to problems. Alcohol dependence syndrome refers to the existence of behavioural and physiological symptoms which indicate a loss of control over consumption and a tendency to continue drinking despite the adverse consequences.
The pathological link with alcohol develops over 5-15 years into an expression of physical and psychological changes, resulting from long-term intoxication. At this time, the changes caused by alcohol are sub-clinical; the psychiatric changes appear later. Traffic accidents are the main problem caused by alcohol intoxication. Others include criminal acts, suicide, accidents and falls resulting in fractures, and traumatic brain injuries. Exposure to the elements can cause freezing or burns. Suppression of the immunological mechanisms may act as a predisposition to infection.
Pathological intoxication is defined by significant behavioural changes, which in most cases includes aggression appearing almost immediately after consuming quantities of alcohol that would be insufficient to cause intoxication in most individuals. The behaviour is untypical in the sense that it does not occur when the individual has not consumed alcohol.
Alcohol withdrawal delirium (delirium tremens) is characterised by disturbance of consciousness, agitation, severe trembling of the hands, frequent micropsic and animal hallucinations (both spontaneous and induced), occupational delirium and autonomic hyperactivity. It is sometimes preceded by generalised convulsive withdrawal seizures. It usually begins on the second or third day after alcohol intake has ceased or has been reduced, although it may occasionally occur beforehand, or after a week of withdrawal. The syndrome usually develops in 2-3 days, unless any other problems arise due to an association with other illnesses. The first episode of this change usually occurs between 5 and 15 years after the establishment of a persistent habit of drinking alcohol. Having a concomitant physical illness may act as a predisposition to this syndrome. Mortality caused by delirium tremens affects 10% of untreated patients, and 25% of patients with concomitant illnesses.
Alcoholic hallucinosis is an organic hallucinosis that develops and resolves rapidly (within around 48 hours) and causes vivid and persistent hallucinations (audio and visual) after alcohol withdrawal or reduction in an individual who appears to be dependent on alcohol but does not experience delirium. The first episode of this change usually occurs after 10 years of excessive alcohol intake. The disorder may last for several weeks or months.
Wernicke’s encephalopathy and Korsakoff’s syndrome are acute or sub acute encephalopathy’s that develop over several years due to a lack of thiamine in patients who are chronic alcoholics. Wernicke’s encephalopathy is categorised clinically by disturbance of consciousness (confusion, apathy, and drowsiness), oculomotor palsy, vertical nystagmus and ataxia. It should be urgently treated with thiamine parenterally to prevent it from developing into Korsakoff’s syndrome. This syndrome is characterised by anterograde and retrograde amnesia, confabulation, changes in attention and concentration, lack of insight into the condition, apathy and indifference. There are changes to the declarative memory (the memory that stores factual knowledge) but not the procedural memory (acquired skills that can be automatically recalled). There are also changes to the episodic memory (for specific events) but not the semantic memory (language, rules, relationships and general principles).
The existence of alcoholic dementia is controversial. Possible aetiology would be the extra vulnerability resulting from thiamine deficiency and the direct neurotoxin effects of alcohol. The characteristic symptoms of aphasia, paraxial and agonise are only observed in one third of cases. The presence of ataxia and peripheral neuropathy is very common. Typical deficiencies are complex reasoning, planning, abstract thinking, judgement, attention and memory. The condition should be diagnosed differently from other types of dementia. Although the deficiencies may remain, they can somewhat improve if sobriety is maintained for several years.
Foetal Alcohol Syndrome (FAS) is an embryo footpath that presents in the children of alcoholic mothers. It is characterised by slow pre and post-natal growth, mental and psychomotor deficiency, facial dysmorphia with microcephaly, microphthalmia or small palpebral fissures, thin upper eyelids, and flattening of the maxillary region. Other malformations include haemangioma, slight abnormalities of the joints and extremities, genital abnormalities, heart defects and single palmer creases5, 8, 9.
Diagnosis of alcoholism
The diagnosis of alcoholism is made using a correct anamnesis, along with a correct clinical examination. It is necessary to assess the quantity and frequency of alcohol intake, and diagnostic questionnaires can be useful for this. A thorough analysis will be conducted using biological markers and an appropriate dual diagnosis of previous or associated psychopathologies.
Diagnostic difficulties are due to epidemiological (the quantity and frequency of drinking) and clinical reasons (problems caused by consumption), excessively rigid diagnostic criteria, absence where there are objective somatic complications, lack of awareness of the illness and denial of the problem by the patient, and trivialization of the problem by the professional.
During the anamnesis and clinical examination, alcohol consumption and its repercussions (behavioural disorders, social, legal and family repercussions) will be assessed. The physical examination will search for somatic changes and other unspecific data indicative of alcohol abuse, and also clinical signs that demonstrate the consequences of alcohol consumption.
Records of alcohol consumption provide data on the duration, frequency and quantity of alcohol intake in each drink. They are highly recommended but little recorded in primary care, where treatment advice on reducing harmful consumption can be useful. The records point towards classic Latin or intermittent Anglo-Saxon patterns of consumption, and also asses risk and non-risk, as well as pathological consumption. Alcohol consumption is measured in grams of alcohol and is calculated by multiplying the millilitres of drink by the strength of the alcohol, then by 0.8 and dividing the result by 100. The term Standard Drink (SD) is increasingly used in Spain to refer to the average alcohol content (in strength and volume) of a normal drink. It is accepted that 1 SD is equivalent to 10 g of alcohol. Drinkers are classified in grades9, 11 according to the amount of alcohol they consume. (Table V).

Various screening tests and diagnostic questionnaires are useful in the detection and diagnosis of alcohol related disorders. Although very useful tools for the detection of harmful consumption and alcoholism, they should be only be used during a structured interview once a final diagnosis has been made, and should not replace the anamnesis and clinical examination. They can be used for:
1. Detection or screening of cases. Help with identifying subjects who fulfil the diagnostic criteria for alcohol abuse or dependence, and whom require additional assessment in order to make a final diagnosis.
2. Diagnosis. Confirm the presence, and where appropriate the severity of the symptoms constituting the dependence.
3. Assessment of the severity and intensity of the dependence. Assessment of the strength of the addiction.
4. Assessment of the cause of the problem.
Biological markers of alcoholism
Physical identification is based on the search for physical stigmata that indicate damage to the organs and systems (flushing, parotid hypertrophy, alopecia and gynoid distribution of body hair etc.). Fundamentally it relates to a series of biological parameters indicative of liver and/or metabolic changes (decreased prothrombin time, increased GGT, GOT, GPT, MCV, uric acid, triglycerides, urea, CDT etc.)
Erythrocyte mean corpuscular volume (MCV) is elevated in 31-96% of alcoholic patients. It is caused by the direct toxic effect of alcohol on red blood cells, folic acid deficiency and associated hepatopathy. Sensitivity varies (25-50%) but increases to 75-80% for outpatients in whom other causes of macrocytosis (anaemia caused by vitamin B12 or folic acid deficiency, hepatopathy, haemolysis, cardiovascular diseases, smoking, haemopathies, hypothyroidism, cytostatics etc.) are not common. Alcohol intake higher than 60g/day increases MCV by 20-60%, resulting in a value of 90-100. Values above 96 are considered to be indicative of alcohol abuse. MCV slowly decreases with abstinence, returning to normal within 3-4 months, and begins to increase again when alcohol is consumed19.
Serum gamma-glutamyl transferase or GGT is the most commonly used marker due to its high sensitivity (65-80%) and ease of determination. Its specificity is lower (50-65%), because it increases with liver disease, cholestatis, pancreatitis, diabetes, obesity, heart failure etc. Therefore 50% of the increase may not be due to alcoholism. GGT is elevated in 34-85% of alcoholic patients and rises when alcohol intake is higher than 40-60 g/day. It is useful when relapse occurs. There is no change following acute alcohol intake, only chronic intake. Prolonged abstinence decreases serum until it reaches normal levels after 2-3 months, but if alcohol intake resumes, levels will rise once again20.
MCV and GGT determination allows over 75% of excessive drinkers to be identified.
GOT or AST (glutamic-oxaloacetic transaminase or aspartate aminotransferase) and GPT or ALT (alanine-aminotransferase) are sensitive but very unspecific (usually indicate hepatic cytolysis). They are elevated in 32-77% of alcoholic patients. 30% of alcoholics present dyslipidemias and hypertriglyceridemia in particular. Uric acid is usually elevated in male alcoholic patients, although it is not very sensitive or specific to alcoholism.
CDT or carbohydrate-deficient transferrin has been used as a laboratory marker since 1992. It is elevated in 80% of heavy drinkers (those who consume between 50-60 g of alcohol/day) and 90% of alcoholic patients. The half-life of CDT is 15 days, therefore its values return to normal after 2 weeks of abstinence and increase again after a relapse if more than 50 g of alcohol/day is consumed during a week. The specificity of CDT is higher than that of other markers. False negatives do occur, given that 18% of chronic and excessive drinkers have a normal CDT level. CDT has a sensitivity of 82% and a specificity of 91.5% with a cut-off point of 20 U/L for men and 26 U/L for women, with better results if the % CDT is used (relative value higher than 6 for both sexes). CDT is used to monitor and control abstinence11.
Treatments for alcoholism
The high prevalence of alcohol consumption and its associated pathologies, as well as the social and work-related problems caused by alcohol dependence, has made the prevention and treatment of alcoholism one of the most important issues in healthcare. Add to this the causal, conceptual and relational complexity of the issue, and it is hardly surprising that treatment options are varied, diverse in their approach, and do not always pursue the same objectives.
There is no such thing as a generic treatment of choice for alcoholism. During the actual development of an addictive disorder as a process, with phases of relapse and withdrawal, it seems most appropriate to focus on the issue of choosing the treatment option best suited to the needs of the patient in each individual circumstance. This broadly covers the concept of not only the immediate but also the medium and long term needs of the patient, within the longitudinal perspective of alcoholism. Very often, treatment options and their criteria for inclusion, exclusion, and also implementation are not clearly defined. In such cases it is of paramount importance to clarify the objective, in terms of the demands, needs, characteristics and the repercussions of consumption, as well as the means available to achieve it.
All treatment programs for alcoholism are generically based on the idea of using pre-motivational exercises which are also continuous and concomitant with various specific programmes. Concepts such as motivational techniques and stages of change are essential for working with alcoholism, regardless of the orientation or treatment option chosen.
A large part of the population of alcohol consumers (which in our culture simply translates as a large part of the population) will not initially require special intense programs, but instead brief preventative interventions. Such actions are usually performed within the Primary Care setting (although they may be applicable in other settings such as occupational medicine etc.). They are generally focused on two groups of subjects; at-risk consumers who have not developed the characteristics of dependence, or any serious consequences as a result of alcohol consumption, and those subjects who have recently suffered consequences, and despite showing the characteristics of dependence, currently have no motivation and relativise their problem in spite of the clear consequences in the areas shown above. The aim of reducing the population’s alcohol consumption to non-problematic or non-pathological levels is pursued using a minimum of information, guidelines and monitoring, as well as facilitating the referral to specialist resources when considered appropriate, according to the criteria. The interventions are simple, easily structured, require minimal training or effort on the part of the agent providing the treatment, and have been successful in different parameters (effectiveness, efficiency, prevention etc.).
In this respect, it should be noted that although for the majority of addictions the final objective is to achieve complete withdrawal (also the aim classically pursued in alcoholism), one should at least consider the validity of an option of controlled and moderate alcohol consumption.
This option could be feasible for middle-aged persons who drink both for social and occupational reasons, as well as by choice, but not to the extent that it damages their health. These programs therefore require a set of minimum inclusion criteria, to guarantee the protection of health in its broadest sense. Candidates would be those who drink in a controlled manner as they “need to” for social and occupational reasons, or those who wish to consume alcohol moderately, and do not present characteristics of moderate to severe dependency, nor physical or psychiatric illness, for which complete withdrawal would be seriously advised. Equally, alcohol consumption should not have caused any social, legal, family or work-related problems. These programs aim to achieve moderate levels of consumption that are non-daily, low quantity, avoid regular and heavy drinking, include periods of withdrawal, and prevent all types of social, physical and psychiatric problems.
Detoxification is usually a necessary step in the treatment for alcoholism, especially in programmes aiming to achieve abstinence. It is understood that this involves the abrupt and controlled removal of alcohol, supported by the use of certain substances to help with the process and avoid the feelings and symptoms of discomfort which may occur (causing the process to fail), as well as preventing physical and/or psychiatric problems which could arise following such a rapid and complete withdrawal. The treatment time is therefore short with clear boundaries, and the only objective is abstinence. Generally it is a necessary step for broader and more comprehensive programs that address the various factors involved in alcohol addiction (cessation programs in the broadest sense). It is rare for patients to report that a program involving the progressive reduction of alcohol until complete abstinence has been successful. It may however be used in a controlled manner by the therapist to begin the process of detoxification when consumption levels are lower. Nevertheless, the treatment resources available today have rendered such methods of prior reduction unnecessary, and short controlled detoxification programs are favoured instead. These programs are planned firstly according to the need for either inpatient or outpatient detoxification. The former is possible if the subject is self-motivated, has good support from friends and family, consumes alcohol moderately without showing signs of suffering from any potentially serious physical or psychiatric illnesses, has not previously had repeated failed attempts as an outpatient, or any history of suffering from delirium tremens or seizures during previous withdrawal periods. The presence of severe withdrawal symptoms is also a contraindication of this type of detoxification.
Chronic alcoholism with marked patterns of moderate to high-level consumption, severe physical or psychiatric effects (including severe previous or concomitant psychiatric disorders) , previous history of failed out-patient detoxification and/or severe withdrawal symptoms (convulsions, delirium etc.) as well as a lack of social support, are all candidates for in-patient treatment. Detoxification programs are carried out by abruptly stopping alcohol consumption (generating the clinical symptoms of withdrawal) and using different substances (depending on the characteristics and needs of the patient) in order to prevent the aforementioned symptoms of withdrawal. Benzodiazepines are the main substances frequently used, along with tiapride and clomethiazol amongst others. Recently noted is the use of other drugs such as oxcarbazepine, gabapentin and topiramate as aids in the detoxification process, and in some cases as the only agent in the treatment process. Detoxification programs usually follow guidelines and protocols which are more or less fixed and standard that guarantee the objective, provided that this allows the necessary flexibility to adapt to each case11,21.
The cessation phase follows the detoxification phase in the treatment of alcoholism. During this period, the main objective is to return to a normal healthy lifestyle and to prevent relapse. Various pharmacological and psychotherapeutic options are useful for achieving this objective, and they are neither inclusive nor exclusive. The main treatment interventions aim to cause an aversion to alcohol consumption, using antidipsotropic drugs or interdictors which act by inhibiting the metabolism of acetaldehyde, and reducing the reinforcement associated with such consumption. They also aim to reduce the intensity of the withdrawal symptoms associated with alcohol consumption that increase the desire to drink, and raise the capacity for control. Various strategies may be implemented on a pharmacological level, in order to prevent relapse. On the one hand, there are interdictors or aversive drugs, including substances like calcium cyanamide and disulfiram, which cause the accumulation of acetaldehyde by inhibiting the acetaldehyde dehydrogenase enzyme, a product of the metabolism of alcohol. This generates unpleasant subjective and physiological sensations for the individual, making relapse difficult. Treatment is therefore based on the negative reinforcement associated with consumption caused by the predictable reaction when alcohol and medication are taken together. The treatment should only be carried out under supervision or in situations where there is a good level of family support. It does not exclude the use of other drugs such as naltrexone or acamprosate. During the last few years, certain options such as acamprosate, naltrexone, topiramate etc. have become popular as they use different mechanisms to reduce the desire to drink, and compulsive drinking. To this same general effect, other substances such as gabapentin, pregabalin, oxcarbazepine, tiapride and certain SSRIs amongst others22, may also contribute. In 2006 the FDA approved the submission of a depot form of naltrexone23. In broad lines it could be concluded that naltrexone was the product that had demonstrated its effectiveness most clearly, particularly in preventing excessive consumption and sustaining controlled drinking programs. It is less useful24 in maintaining abstinence. Its effectiveness does increase significantly when used in conjunction with psychotherapy programs, especially those which include cognitive-behavioural therapy25. A proposed clinical intervention would come from the need to consider the profiles of patients who would most benefit from each of the drugs and programs19, 26.
Almost all of the schools of psychology offer treatment and rehabilitation programs for alcoholism. The basic structure of a cognitive-behavioural treatment program for alcoholism includes the use of motivational techniques to encourage abstinence and prevent relapse, as well as a series of tools aimed at both coping with withdrawal symptoms as well as the factors that have caused the individual to start drinking and develop a regular habit of consuming alcohol. The techniques employed include help with conflict resolution, coping with anxiety and depression, and developing social skills, amongst others. Many of the treatment options are combined with, or are carried out exclusively using group work. This type of treatment, led by professional therapists aims to achieve specific objectives through group participation. It differs from other treatment options which are also useful for alcohol cessation, such as self-help groups generally made up of alcoholics and ex-alcoholics who share their experiences and support each other in their aim to achieve abstinence and recovery.
For certain individuals in some situations it may be difficult to achieve complete alcohol withdrawal and cessation, either due to lack of will power and / or motivation on the part of the patient, lack of psycho-social support, or difficulties caused by medical or psychiatric pathologies. In such cases it may be useful, at least initially, to consider harm reduction and disease prevention programs. These types of programs which have already proved their usefulness and effectiveness in different settings are modelled on the harm reduction programs already in place to deal with the HIV epidemic. Initially, the aim is to encourage minimal contact with the healthcare environment, subsequently allowing for more ambitious goals to be set. These types of programs try to teach the patient how to reduce their alcohol consumption, and act on the problems and other factors associated with drinking27.
In relation to the psychiatric pathologies resulting from alcohol abuse and alcohol dependence, the first treatment option is total withdrawal. Alcohol consumption alone can cause its own cycles of depression and anxiety, as well as psychotic episodes, which may improve or even disappear with abstinence. For those episodes that persist even after withdrawal, the use of anti-depressant drugs (SSRIs, TCAs etc.) and anxiolytics is proposed, whilst trying to avoid drugs such as benzodiazepines as they carry the risk of substance abuse. Other psychiatric pathologies require more specific approaches27.
Finally, it should be noted that specific preventative programs are required in order to deal with some of the particularly problematic issues associated with alcohol use, such as the increased risk of suicide amongst alcohol consumers.
