Subscription contract

I am not postive, as you can see, but since I can't find a direct reference, this is what I think of. It sounds like a form to be filled, asking for a subscription to the service, and the paper stands for a contract between both parties, the provider and the customer. Is it possible that this was translated from another language into French?
