...and once that is done, you might just add a little context such as the background to the form; it does appear to be a Sheriff application to a Scottish court, but that IS only a guess. So what does the form "humbl(e?)y sheweth!?

Karl, where has your upper case key gone? If YOU can't understand the sentence, and it is in front of you, it is much harder for us. I'd suggest enetering it EXACTLY as the text shows - in capitals if that's how it appears - with a corresponding note....
