...la circunstancia de coincidir el órgano diana y el órgano transplantado

Las infecciones representan un reto en el seguimiento del paciente receptor de un transplante. La inmunosupresión obligada supone un mayor riesgo de infecciones bacterianas, víricas y fúngicas, la mayor respiratorias. En el trasplante pulmonar se da la circunstancia de coincidir el órgano diana y el órgano trasplantado, con el consiguiente peligro de rechazo de dicho órgano.

OK, the text is about treating a fungal infection in a lung transplant recipient. My question is about "... se da la circunstancia de coincidir..." Surely in all organ transplants the donor organ must match... Also it is a bilateral transplant, so it's not like the donor organ has to match an existing lung... what am I missing here?

Help greatly appreciated, particularly from those with knowledge of the field.

right, here goes: In lung transplantation, it so happens that the transplanted organ is also the target of the infection; this implies a greater risk of the lung being rejected.

MJ, my understanding is that "el órgano diana", that is, the target organ, is the target of the infection. In other words, the infection (as the text says is mainly respiratory) targets the lungs, in this case also the organ transplanted.

It certainly does seem contradictory: I presumed it was a problem that the donor organ was alongside the other original lung, but if you say that it was a double transplant, then this isn't the case... Hmm
