Reference: For alignment Reference information: I have withdrawn my answer, since it duplicates Virginia's, which was posted first. Here is some of what I said: In the top section, known as the "Carrier block", of the 1500 Health Insurance Claim Form, "PICA" appears on either side, followed by three boxes, just above the thick horizontal line that separates this section from the Patient and Insured Information section. PICA here has its typographic meaning. The three boxes are called PICA alignment blocks. Each block is 1 pica wide. A pica is a typographical unit, which is almost exactly 1/6 inch. There are different types of picas. But the idea of these blocks is to ensure that the form is correctly aligned when it is filled in, since it will be typed or printed and then needs to be read by a scanner. That's all it means. "The National Uniform Claims Committee (NUCC) has approved the printing standards for Form CMS-1500 (08-05) paper claim. These standards are as follows: The Form CMS-1500 (08-05) is designed to accommodate 10-pitch Pica type, 6 lines per inch vertical and 10 characters per inch (cpi) horizontal.
