The XXX femoral stem is a cementless implant that combines the advantages of a straight stem geometry, giving a more accurate fit in the diaphyseal region, with that of an anatomic metaphyseal and extramedullary design to increase proximal load transfer, range of motion and joint stability. The anteversion, neck length and offset of the stem gradually increase with the size of the implants, closely replicating the anatomy. The XXX stem has a constant stem-neck angle of 128° and is available in 7 different sizes for both right and left femurs. For a given resection level, increasing the implant size will increase the offset and the anteversion without lengthening the limb.

Pre-operative planning is an important step prior to total hip surgery. X-ray templates are available in different magnification for each stem size both in the AP and axial view. (fig. a) It is first recommended to assess the magnification of the radiograph and use the appropriate templates. The use of a calibration device on a pre-operative x-ray can help determine the x-ray magnification. However, the magnification is not a constant value due to variables such as patient size and positioning. The final implant size should, therefore, be determined intra-operatively. The estimated size of the implant, neck resection level, centre of rotation, femoral offset and leg length are all determined on the pelvic A/P radiograph.
