The imbalance theory focuses exclusively on prostacyclin as an antiplatelet agent when in fact there are multiple lines of defense to clotting in an artery. The first line of defense to a thrombotic occlusion is comprised of a system of anticoagulant proteins that circulate in the blood, including antithrombin III, heparin co-factor II, and activated Protein C (APC). The second line of defense includes a group of short-lived, endothelial derived platelet inhibitors, including nitric oxide, prostacyclin, and ADPase. The third line of defense is comprised of the components of the plasminogen activator system, which under normal circumstances is relatively quiescent.
However, in the presence of a clot, this system is catalytically activated and ultimately targets the protease plasmin to the fibrin (clot) surface, which leads to clot dissolution and clearance from the circulation.
