How do I request an exception to the XXXXX formulary?

I was thinking "exepción" but when I look at the entire sentence it seems awkward. I was thinking "exención" or "rectificación" but even then -awkward!

Suena rebuscado.

What would you all use in this case?

It has to do with a prescription drug formulary this organization has. The client can only use the medications that are preferred by this organization. Sometimes, what happens is that a person needs a certain type of medication but it isn't found on the formulary. The person wants to know how to submit for an exception to the formulary. In other words, allow them to purchase the medication at the co-pay price even though it is not on the list.
