Genealogy reports include personal information, such as addresses and telephone numbers, and are considered confidential by X. Any distributor who wishes to request a genealogy report (either in hard copy form or from the Internet) must read the Genealogy Report Non-disclosure Agreement below and understand the terms under which he or she will be provided this information. If the distributor agrees to these terms, he or she must sign and date the bottom of this form and fax or mail it to his or her local X-office. By entering into the Non-disclosure Agreement, the distributor agrees that it will be regarded as a data processor and will comply with the Data Protection Act 1998 and any regulations made under it, as if it were a data controller as well as the instructions given by X. If the distributor wishes to be able to access this information on the Internet he or she must provide X with a desired PIN, which will be activated upon receipt and acceptance of this form.
