(shitsureidesuga) onamae ha?

Depending on how old you are and how polite you want to sound, you can skip shitsureidesuga. A popular Japanese text book in Australia for junior high students says only "onamaeha?"

Onamae wo oshietekudasai.

This way could be the very first time phrasing for you to ask somebody to know his/her name.

onamae wa?

the way that the word is spelt is the way it is said. to break it down, all vowels without a constanant is spoken by itself, so onamae wa would be said: o na ma e wa. 'o' is said as in the 'o' in hot and spot, where as the 'e' is said as in the 'e' in bet and get. I hope this is understandable.
