Considering the context, it seems to me that जमाना should actually be जनाना. Then, जनाना सकूनात could possibly mean maternity home, and जनाना सकूनात शहर would be city where the maternity home is located.
The meaning of मुतसिल मकान is less controversial - it clearly means adjacent house as मुतसिल in Urdu means nearby or close-by or adjacent.
More context will help. For example, which state has issued this birth certificate? In India, the only state where Urdu is officially used in Kashmir. Or is this a birth certificate issued by another country (Pakistan?)?
