I know this may sound a bit wacky, but I'm sure this is what it means. These are Colombian identification details. C.C. is obviously Cédula de Ciudadanía, the basic ID document. L.M. is libreta militar, associated with military service, and D.M. goes with it and means distrito militar. These details are routinely included in CVs and official certifications even in the academic world.

"Los aspirantes a grado deben entregar en la Secretaría Académica la siguiente documentación:
1. Recibo de consignación, pago Derechos de Grado equivalentes al valor establecido anualmente en los valores de derechos pecuniarios que se publican en la página de la universidad ($98.250). Diligenciar con código 50.
2. Fotocopia de la cédula de ciudadanía ampliada al 150%. Presentar original.
3. Fotocopia de libreta militar ampliada al 150%. Presentar original (varones). [...]"

OK, this doesn't explain D.M., but it's obviously associated with the libreta militar, and here it's spelled out:

"ANTONIO GALAN SARMIENTO, mayor de edad, vecino de esta ciudad, identificado con la cédula de ciudadanía número 13.814.653 de Bucaramanga y Libreta Militar número D 242069, Distrito Militar número 32, quien obra como Gerente y Representante Legal de la Empresa de Telecomunicaciones de SantaFe de Bogotá [...]"

In your document it appears that L.M. No. and DM have been left blank, presumably because the person doesn't have a libreta militar. I understand it's not compulsory now in order to graduate.

Thanks for the comment, Christian. That's certainly possible, but it doesn't affect the meaning of DM; it would simply explain why it has been left blank.
