"spike" es sinónimo de "pierce",
las chemo bags tienen por lo general un tapón de goma donde se inserta una aguja o un dispositivo para el goteo. Se me ocurre que esto se puede referir a bolsas que ya se hayan sacado de donde estaban (un paciente o algún equipo) y puedan tener el tapón perforado o pinchado.
No se me ocurre una palabra para traducir "unspiked" pero la idea en este contexto sería "usadas", "utilizadas", "descartadas", "con el tapón perforado".
Espero que alguien más te pueda dar una mano.

3.3 All final parenteral chemotherapy medications need to be spiked with a buretrol or
non-PVC tubing set in the chemo hood, excluding IV push syringes, leucovorin, and
drugs that will not be spiked prior to administration (intra-arterial & intrathecal).
3.4 Upon completing the preparation process, the final product is labeled. All vials,
minibags and/or syringes used in the preparation of the dose must be checked by a
pharmacist. The syringes for each drug and/or solution used in preparing the product
(including syringes used to dilute drug vials) must be pulled back to indicate the
measured volume. Needles must be recapped and left on the syringes before placing in
the bin.3

f you're administering chemotherapy in a hospital, it's likely that the drugs used will be prepared in the pharmacy. IV bags with chemotherapy drugs must be spiked in a biological safety cabinet or hood. To prevent having to spike bags at the bedside, new tubing should be connected to each bag of chemotherapy before it's sent to the nursing unit. IV tubing should also be primed with a non-drug solution before the chemotherapy drug is added, or a back-flow closed system should be used.2

If at any point you need to transport chemotherapy, make sure you do so properly. The chemotherapy bags should be placed in another bag and appropriately labeled as chemotherapeutic agents. Many facilities use a hard-plastic carrying case for these drugs. A pneumatic tube system should never be used to transport them.

Cuando los viales sean liofilizados se deben utilizar siempre los filtros de venteo; y cuando no sean
liofilizados en algunos por la clase de tapón que tienen el laboratorio recomienda la no-utilización de
filtros, entonces se pueden introducir dos agujas o lo que es mas seguro, introducir una sola aguja
metiendo en el vial menos aire de la dosis que tenemos que sacar para evitar rebosamiento.
