: Perish here does not mean to die, if you refer to the dictionary meaning of "perish", you face to this meaning: "to die, especially in a terrible or sudden way"

and is bound to be wasted/consumed/spent
You can look the meanings up in Random House Webster's Unabridged Dic.
The words used here are a bit archaic and mostly used in Bible text.
However, if you want a more-easy-to-understand expression, just go for things like:
and is dying/ is bound to die/pass away.

: this captures it, I think...it doesn't mean he is dying soon, but rather his mind is going and will never be the same again

Discussion
to cut down all expectations
there is no hope for him to become a normal man again
My explanatio:
First: از دست رفتن means مردن هلاک شدن بی اختیار شدن according to
Second: according to Bateni's Dictionary
Perish means 1
I suppose you can use "perish" in this part of its meaning
َMy other reason to use "perish" is its literary sense.
http://www.thefreedictionary.com/perish
از دست رفتن here does not mean die or pass from existence. What do you think?
