I guess this document is "meeting minutes" but perhaps there's a better technical term.

The document itself seems to be a description of a meeting.

meeting or interview? It depends on the type of gesprek? If it is related to job performance, I would say it was an interview report.

Meeting (minutes) report I agree with Chris that 'meeting minutes' seems fine here. Other options would be 'meeting report' or 'meeting minutes report'.

minutes I'd say 'minutes' was right here; a gespreksverslag generally takes the same form as notulen in that it's a point-by-point formal summary of the discussions and decisions of a meeting (as opposed to a transcript, which is a literal, word-for-word record, or a précis, which is a concise account).

Record of a discussion Minutes of a meeting would be a bit more formal. "Record of a discussion" is a bit less threatening.
