No sé si se trata de alguna cirugía "cosmética" o plástica, pero el self-pay refiere a la parte a cargo del interesado (copago)

Explanation:
Imagino que se trata de un centro donde hacen operaciones donde se puede combinar la parte sanitaria con la estética, tipo por ejemplo operaciones de nariz o de pecho. Así que si tienes una prescripción del médico para esa operación, parte o toda la operación puede ir cubierta por el seguro que tengas, según el médico haya escrito la prescripción, y el resto de la operación se la tiene que costear el paciente.

Ejemplo de operación de nariz:
SOLO SEGURO: Enderezar el septo nasal.
SEGURO MÁS PRIVADO: Enderezar el septo nasal y eliminar pico aguileño.
