הפניית שיחה לעומת העברת שיחה

Hebrew to English Other Telecom(munications) telecommunication systems
Is there a real difference between the two?
Which of them is better for translating "call forwarding"?

IMHO they are very different הפנית שיחה is call forwarding. Unless I am mistaken, which I may very well be, העברת שיחה is transferring a call. The difference is that in the case of call forwarding, you do not get through to the number you dialled. The call is automatically routed to another number. In the case of call transfer, you do get through to the number you called, but it is determined that you need to speak to someone else, and the person you are speaking to - who is the person you have dialled - can put you through or forward you to the person you need to speak to.
