Buongiorno Giorgio, credo che dipende dal metodo d'inserimento dei dati previsto dall'autore. In questo modo la persona che compila il modulo scrive direttamente tutte le finali che riguardano il suo genere, inserendo Il/La dovrà scegliere il termine e le finali corrette tramite sottolineatura. Credo che basti scegliere uno dei due metodi per fare un modulo coerente.

Sono d'accordo, però avrebbe dovuto essere "Il/La". Probabilmente è stato inserito male.
