course of studies report
Explanation:

  -> dzieki!

neutral  Konrad Talmont-Kaminski: This is quite awkward, I think. The short version could mean anything.

  -> come to think of it.. so it is! Jan Stanislawski stands corrected (but then again, not for the first time: http://www.serwistlumacza.com/texts/stanisl.html ). As to short version -yes, it could mean anything. So could "index" in Polish.

Znam to z pracy, gdzie trainee doctors musza przedstawic swoje trainee's logbooks, zeby zaliczyc rok. stad dla undergrads uznalam student's logbook bedzie wlasciwa analogia.

agree  Konrad Talmont-Kaminski: This may work when speaking to Poles but someone from the UK will not understand.

  -> yup, that's what I thought:) isn't it the same with words like 'glottodidactics' and 'philology' itself..?;)

Course record

Explanation:
The 'Index' does not exist in the UK or in other English language unis. What students get are computer generated print-outs which list their courses. These are referred to as course records. If it is important that you are dealing with a physical booklet, call it a 'course record booklet'.

agree  Michał Wiśniewski: Konrad, booklet doesn't sound very official, though, perhaps 'course record book'?
11 hrs
  -> Yes, you're right - that is a problem. However, it can also signify its small size. 'Course record book' sounds large and heavy.

Explanation:
Kiedyś na zajęciach z "legal translation" spędziliśmy prawie godzinę próbując znaleźć odpowiednie tłumaczenie naszego "indeksu". To jedne z propozycji. Może się pzydadzą.
