Brasil
Seguindo o modelo herdado do Exército Português, inicialmente, a primeira patente de oficial do Exército Brasileiro era a de alferes. Na reforma das Forças Armadas de 1930, sob comando de Getúlio Vargas, a patente foi substituída pela de segundo-tenente.
Estados Unidos
Até 1800, existiam os postos de ensign (alferes) e de cornet (corneta) no Exército dos Estados Unidos, que correspondiam à primeira patente de oficial, respetivamente, das armas a pé e da cavalaria. Nesse ano, ambos os postos foram substituídos pelo de second-lieutenant (segundo-tenente).
Na Marinha, Guarda Costeira e outros serviços uniformizados de âmbito marítimo dos Estados Unidos, existe o posto de ensign que corresponde à menor patente de oficial. Equivale ao posto de segundo-tenente do Exército, Marines e Força Aérea. O posto de ensign foi introduzido na Marinha dos Estados Unidos em 1862, em substituição do de passed midshipman (guarda-marinha aprovado).

Ensign stands for standard or standard-bearer and was, therefore, the rank given to the junior officer who carried, or was responsible for, the flag in battle. This rank has generally been replaced in Army ranks by Second lieutenant. Ensigns were generally the lowest ranking commissioned officer, except where the rank of subaltern itself existed.[4]
