At one time I used to do these for the courts. They can have several names, but this was one of the names we used for them (Texas, USA).

Adoption Social Worker Report
I am also translating an adoption file and this is the term I was planning to use. It seems quite clear and unambiguous.
"Adoption social worker completes Adoption Social Worker Report (Word,49.5KB), Adoption social worker returns form to LAC Team..."

I really think that the 'home study' is only a part of the social worker report. In the adoption file I am translating, there is a reference to a 'visita domiciliaria' which is one part of the 'estudio social' and the 'informe social'...I believe 'visita domiciliaria' is the home study.
