In this team, there is also evidence of co-working between the professions and others to maximise the chances of success; for example, the joint family work initiated by the nurse and supplemented by the occupational therapist, both team members working closely to ensure they were consistent in their approach and that family sessions related to issues identified by both of them. Further examples include the reinforcement of cognitive strategies taught by the nurse during occupational therapy sessions, the careful management of the medication’s positive and negative effects by the medical and nursing team and the future planning for Carl’s hoped-for return to university between the occupational therapist, the student welfare department and a personal tutor. The ward team had first to liaise with the crisis team who were first to respond and assess Carl’s situation and the early intervention team became part of Carl’s treatment team long before he was discharged. All the team members across the services worked closely together, in collaboration with Carl, in exploring future options and planning to meet his needs in order that he might be able to move on from the hospital as quickly as possible. In this instance the seamless service aspired to in the guidance documents was evident and Carl’s experience followed an optimal pathway (Birchwood et al 2001).
