This will indicate the make-up of how you plan to leave the dock.
a. Tow And Terminal Gear To Be Used Was Examined And
Found Satisfactory___
Inspect all gear to be used during the trip including push gear, tow gear,
shackles, lines, etc. before and after make up. Ensure all is satisfactory for
the intended voyage and check space provided. If not satisfactory, advise
dispatch of the condition.
b. Transiting Make-Up
PUSHING___ ALONGSIDE___ ALONGSIDE STERN FIRST___
TOWING
You should indicate here if you plan to change the make-up for your
transit once you are away from the dock.

This is simply a questionaire concerning how the owners or master of a vessel prefer to be manouvered upon departure from the berth.Some vessels have sensitive parts on the ship, like they prefer to be towed instead of pushed, if they have weak spots in the hull plating for eaxample.
