in this case "faire état" means indeed to state or to declare. The "faux ami" is "qualité". It has nothing to do with quality as such but refers to his capacity or competence in this case.
The doctor is simply requesting official permission to use the title of "surgeon"

"Faire état de" means to state, report or (maybe in this context) declare. So, here it means stating (or maybe making a statement on) the quality of specialist qualified in general surgery.

Absolutely right. Something like "capacity" would be more appropriate. But CMJ_Trans has nailed it, I think, and if I were the asker, I'd be giving him the points. I certainly don't deserve them after such a silly mistake!

: It's not the doctor's condition, it's his profession. The use of condition relates more to a state of being, or a circumstance.
