I think this has got to mean "Jefe del Registro del Estado Familiar". "REF" is a standard abbreviation for "Registro del Estado Familiar" in El Salvador. Alternatively, it's just possible "Ref. is a typo for "Reg." and "Estado" has been omitted. Either way, I'm sure this is what it must be.

I would not recommend reducing this to "head of the office of vital statistics" or "civil registry"; it's more specific than that and is a particular Salvadorean institution.
