Explanation:
OK, in translating this from French to English, we have to take a little bit of liberty. In English, the toes are numbered 1, 2, 3, 4 and 5, the big toe being #1. The 1st toe has only two phalanges, but the others have three each, the proximal, the middle and the distal phalanges corresponding to the French "premiere", "deuxieme" and "troisieme" phalanges. So, the French text says that there was an amputation of the proximal phalanx and the middle and distal phalanges of the fourth toe. The beginning of the sentence was not included, so perhaps there is some modifier of what happened to the proximal phalanx which would explain the somewhat clumsy sentence structure. Additionally, since surgeons do not generally fillet out the bones of the toe, I must assume that the attached soft tissues of the toe were also amputated (parenthetical smile).
Assuming that the deleted first part of the sentence does not specify otherwise, one could simply say that the 4th toe was amputated. Incidentally, the "ray" also includes the metatarsal bone, but from the context, it does not appear to have been affected.

BTW, correct me if you disagree, but I really do not see any medical reason in the source text for such confusing language other than the usual penchant of French academic prose for obfuscation and elitism. :-p
