¿Qué tal?

Several possibilities depending on context and familiarity with the person we are addressing:

I would translate "today" only if the context suggest that the condition of the person may have evolved:

¿Cómo se encuentra hoy? (formal - a nurse to a patient)

¿Qué tal estas hoy? (familiar - someone to a friend that was not feeling well yesterday)

Other than that, the word "today" suggests a certain degree of familiarity, so I would use:

¿Qué tal? (informal/formal polite formula in passing or when introduced to someone - we don't really want to know)

¿Qué tal estas? (informal) or ¿Que tal está? (formal) a little bit more keen in getting an answer

¿Cómo está Ud? (polite formula after being formally introduced to someone)

Como estas hoy?

Unfortunately, I am unable to insert accents in the answer box, so you will need to put an "/" accent over the first "o" in "como" and over the "a" in "estas". Also the phrase should start with an inverted question mark.

This is the way to address a friend or relation; otherwise, the formal greeting would be "Como esta usted hoy?", with accents in same places and also the inverted question mark as above.

Alternatively "Que tal hoy?" with accent on "e" of "que" and the inverted question mark as above.
