I think the confusion occurs in the translation of the word مستلزمات which, I believe, should be 'requirements'. Once it is translated as such the sentence makes sense.
"Order form for the release of medical, dental and factory requirements and devices."
In this case factory is a mass noun in English, as to pluralise it would make for poor English expression.

تأكد من وجود" المصانع" في النص الذي لديك ، ربما تكون كلمة أخرى, يوجد بعض الشذوذ في السياق، إذا كانت المصانع" ذكرت في محلها ما المانع أن تترجمها " المصانع"
