In the record maintained by a municipal government relating to an application by a citizen for a passport, after Wohnort and Ortszusatz, the field "O/K-Name" appears. The field after the word "O/K-Name" is blank, so I don't have more clues.
"O" may mean "Ort", and "K" may mean "Kreis", but I am not finding anything for "O/K".
