老弟,你抠得太细了,当然trauma可指精神创伤,但这儿physical trauma就是指我们一般所说的外伤。

英文词汇的意义大多是多重的,您最好根据上下文理解、选择词汇。在中文意义上外伤其实就是指身体外伤,一般不会误解为精神创伤。:)就您所举内脏出血的例子,其实在医学上因外力引起的内脏受伤,也称为外伤。
如:腎臟外傷依受傷的情形分為鈍器式及穿刺式二種。有百分之八十以上的腎臟外傷是屬於鈍器式外傷,是指經由外力或高處跌落而扯及或直接撞擊到腎臟。有時候,因為外力太強而牽扯到腎血管,造成血管斷裂,也是會引起這類的腎臟傷害。而穿刺式外傷的比例比較少,大約在百分之十之下,通常患者因為槍傷、刀傷等直接穿刺到腎臟所致,屬於較嚴重性的外傷,值得注意的是否合併有其他器官的外傷。

英文有個字,trauma, 也就是重創的經驗,久久不能平復, 帶來心理與身體上的受創. 美國國家心理健康學會National Institute of Mental Health 給 trauma的定義是: trauma means experience that is emotionally painful, distressful or shocking, which often results in lasting mental and physical effects.

經歷 trauma的人, 需要時間復原,也需要親人的支持. 美國在九一一事件之後,學校的心理諮商師學會 National Association of School Psychologists (NASP),提供給家長老師一些建議,教導大人如何協助孩子面對 trauma. 最近在大華府地區的 sniper shootings 事件, 又重新讓人想起過去的 trauma.
