Alle Liebe zum Geburtstag wünschen dir Ivon und Heather
Instead of "dir" you could even use "Ihnen" (which is the German form of courtesy, if you are not close friends)

Alles Gute zum Geburtstag wünschen dir, lieber Hans, Heather und Ivon
I personally would add the name of the person as an apposition. It is more personal. I would also prefer "Alles Gute".
Reference:

Alles Gute zum 81. Geburtstag wuenschen Dir Ivon und Heather
"You" ("Dir") and all its German variations should be capitalized in letters and greeting cards. If you want to mention the birthday year, this is the way to add that to what other translators have suggested.
