This is in the context of a report by a Labour Inspector about an employer's alleged failure to consult the Works Committee about planned redundancies. The Inspector wrote both a "procès-verbal" and a "signalement" which was then sent to the public prosecutor. So far I have used "report" for "procès-verbal" but am unsure how to translate "signalement". See below:
"II. SUR LA NULLITE DU SIGNALEMENT
1. Madame STEINBERG, Inspectrice du Travail à PARIS, suivant signalement en date du 13 Avril 2001, complété le 9 Octobre 2001, a prétendu que, Messieurs XXX et YYY, notamment se seraient rendus coupables du délit d’entrave au fonctionnement régulier du Comité Central d’Entreprise de ZZZZZ.
2. L’Inspectrice du Travail n’a ni adressé, ni remis, par lettre recommandée avec accusé de réception, ou par courrier simple, un exemplaire de son signalement et de son complément à Messieurs XXXX et YYY ; de surcroît, l’Inspectrice du Travail a transmis ces signalement et complément directement au Procureur de la République de PARIS, sans suivre la voie hiérarchique.".
Could it be something like "report of an offence"?

As "signalement" means precisely the reporting of an offence to the appropriate authorities, I don't think there is any way you can escape the use of the word "report" if you are going to be accurate. However, you can perhaps turn your sentence round a bit, perhaps more easily in the first instance than the second.
