The diary card is not very patient-friendly...
Explanation is self-evident, I believe (akin to user-friendly). However, there is a glaring grammar mistake in the German: ...Ausfüllanleitung MUSS den Patienten.. or Ausfüllanleitung IST den Patienten verfügbar zu machen.
