I am continuing to work on a project involving the manufacturing of medical device labelling. Here is the sentence that is giving me some trouble:

Completato l’iter di codifica/modifica, alla ricezione del 1° lotto di IfU o Blister da parte del fornitore viene compilato lo stampato n° 008 (allegato n°5) ***apponendovi*** un campione originale e le approvazioni di Int’l Mktg, RA, QA e R&D.

How would you translate apponendovi here? simply "affixing" or "putting"?

I think I have been staring at it too long!

agree Kimberly Wastler : Attaching or some other form of "to attach" is fine (you may want to change word order). A specimen (blister) is going to be attached (probably stapled for prosperities sake) to Attachment 5 (which is form 008) which will have the approvals on it.
