You are right, the word fire covers both meanings. To make the distinction, I suggest you use "conflagration" the second time.
An unextinguished fire can be the cause of a conflagration.

: "Conflagration" brings the connotation of a disaster making headlines, like "California Conflagration of September 17, 1923". I haven't seen it used in the context of safety regulations/procedures, etc.

: По-моему "failure to put out" - отличный вариант, звучит очень естественно... а также думаю, стоит уточнить, что это campfire/bonfire. Not sure about "ignition/inflammation"... how about "a blaze"? Not easy though!
