This is a very formal document recording the minutes of a meeting. The full sentence is "Por parte del xxx, se solicitó la inclusión de una intervención realizada en relación con el caso xxx, que no quedó en el acta". (referring to a previous record of an earlier meeting) My try: On behalf of the xxx, the inclusion of participation/involvement realized/carried out/effected in relation to the xxx Case was requested, which did not appear in the document."

: "intervention" is suitably vague -- which is a good idea when there is insufficient context, as here. "Statement", "meeting", etc. are much too specific.
