Spanish term

para que el efecto se tuvo a la vista

Spanish to English Medical Medical (general) Endocrinology
The complete phrase is:
Por este medio se hace constar que para el efecto se tuvo a la vista la historica clinica No. 999999 correspondiente a: [nombre de persona]

I've gotten as far as "It is hereby certified that" or "I hereby certify that" - and I understand the overall idea that the letter is to certify that the person named was seen at the clinic. However, I've gotten lost in the verbiage, and want to make sure I don't miss something important.

Thanks in advance!

to the effect, clinical history number 999999 was taken into account

As well as when "to all effects" is used to translate "a todos los efectos", in this case, the singular is used (such and such is taken into account to ONE single effect).
