BB-hemvård would be if the BB staff care for you in your home after the birth - probably by telephone support. This would probably be for those discharged within the first day after birth. If it is only telephone support offered you might want to use a different term to "home care" (as this perhaps implies visit)
BB mottagningen would be a clinic for new mothers to go to after discharge from hospital

reply to Deane's comment: I think that in reality "home care" does not include many home visits - home visits are possible but usually it is support by phone that is given as "home care". The "home" refers more to the mother being discharged home (relatively early) after birth but still being under the care/responsibility of BB (or part of BB).
In Sörmland it is described like this: På BB-hemvård jobbar barnmorskor året om. Här kan vi hjälpa dig med:
•Besök hos barnläkare
•Provtagning
•Hörselundersökning av ditt barn
•Rådgivning och samtal
•Amningsrådgivning
Efter förlossningen måste du och ditt nyfödda barn stanna kvar i minst sex timmar på sjukhuset. Under den tiden ska ditt barn undersökas av en barnläkare. Efteråt kan ni välja att åka hem eller att förflyttas till BB. Barnet måste vara friskt för att ni ska kunna åka hem.
En barnmorska från BB-hemvård kommer att kontakta dig när du kommit hemma igen. Detta gör vi för att se till att du och ditt barn mår bra. Om du vill så kan vi besöka dig hemma. Du är alltid välkommen att ringa oss om du några frågor eller funderingar.

In Jönköping: BB och BB-hemvård
Efter förlossningen kan du i samråd med personalen planera att gå hem tidigast efter 6 tim.
Detta förutsätter att du och ditt barn mår bra och att barnet är undersökt av barnläkare vilket endast sker dagtid.
Normal vårdtid är 6-48 timmar, men individuella skillnader förekommer.
Ni följs upp med dagliga telefonkontakter och återbesök.
Du kan när som helst kontakta BB-hemvård.

this may differ between regions depending on the health care budget....

: Agree, but home care includes a midwife actually visiting at home after birth = postnatal. Well, I recall going to the hospital clinic prior to birth, but it seems BB mottagning is only postnatal, too! And yes, I changed my mind about neonatal.

While this is only in Swe -- it might be interesting reading for you in relation to all these questions you have posted.
Now I see Tania's references, so this will be a supplement. (Aren't we being helpful!)
The only comment I have to Tania's remark about 'home care' being telephone based is that here in Uppsala it involved the midwife coming home to visit, where these visits were planned prior to us leaving the 'maternity ward' with our newborn. This involved more than one visit over the first month or so. When everything seemed ok this tapered off quickly with referral to the local medical clinic -- which housed the midwife clinic. Otherwise, we were referred to the hospital outpatient clinic (the BB mottagningen) for specialist MDs.
But, then here is the same service described -- more like Tania's reference. Tho their cars weren't as good looking back then...
