(Wouldn't it be nice if doctors wrote in whole sentences...)
Testing a quadriplegic patient's motor control:

Au MID: en proximal les stabilisateurs fessiers sont à peine à 3, le quadriceps et à 4/4+, les IJ sont à 3 quelle que soit la course testée. En distal: JA, EC, PL, JP, triceps contre pesanteur ou un peu plus, EP>4.

My hunch is that "un peu plus" means that the triceps score 4, or perhaps a bit more, but the word order doesn't support that idea. The "contre pesanteur" bit has also got me puzzled.

triceps surae (gastrocnemius and soleus) against gravity or a little better
Explanation:
The grade applies also to the muscles before "triceps surae" - so they are grade 3-4, or 4- (4 minus)

http://www.neuroexam.com/content.php?p=29

Patterns of weakness can help localize a lesion to a particular cortical or white matter region, spinal cord level, nerve root, peripheral nerve, or muscle. Test the strength of each muscle group and record it in a systematic fashion. It is wise to pair the testing of each muscle group immediately with testing of its contralateral counterpart to enhance detection of any asymmetries. Muscle strength is often rated on a scale of 0/5 to 5/5 as follows:

0/5: no contraction
1/5: muscle flicker, but no movement
2/5: movement possible, but not against gravity (test the joint in its horizontal plane)
3/5: movement possible against gravity, but not against resistance by the examiner
4/5: movement possible against some resistance by the examiner (sometimes this category is subdivided further into 4–/5, 4/5, and 4+/5)
5/5: normal strength

IOW for the list of muscles between JA (tibialis anterior) and triceps (t. surae), muscle movement is possible against gravity and/or with a very slight amount of resistance.

triceps strength: able to hold up arm or a bit more

Explanation:
Testing triceps strength would involve asking a person to hold their arm out without letting it drop to the side. Then the physician would put pressure on the arm to see how much more weight the triceps could hold besides the weight of the arm.

you are right-----I had not noticed the MID------in English, although triceps could refer to any muscle with three heads, it almost always means the triceps brachii (in the arm) and the triceps surae would be named as such. but doctors don't follow the rules!
