I think you can say "medical card" or "health card".
That is a health certificate which is requested for some jobs in Argentina.
I think we also needed that in primary school, but many years have elapse since then...

Libreta Sanitaria
Child's health book
Jenette: the "Libreta Sanitaria" was a booklet given to the child's mother when the child was born. I say "was" because since 1989 it has been called "Libreta Sanitaria Materno-Infantil" and it includes other data about the mother. The Libreta Sanitaria records all data referring to the child's birth: Date, place, physician's name, whether the birth was natural or pro caesarea, weight, length of the baby. Also all visits to the paediatrician are recorded and vaccines through the age of 12 approximately. I think "Child's health book" is the most accurate way to put it.
