This occurs in a discussion (written in Vienna) of a padded face mask to support the head of an unconscious patient in the prone position (i.e. face down). The mask leaves parts of the head free (e.g. eyes, nose, mouth) while supporting it elsewhere.
It seems to me that the parts that are "frei" are not "nach unten gelagert" - have I misunderstood it? Should "nach" be "nicht"?

From your discussion, I would think similar to mbrodie that there are the open portions of the mask that are not covered by the mask (open) but which are still supported from below. I think the support can come from the side or diagonally onto the face so that, while the face IS supported from below, it is still open.

I agree with mbrodie and Trudy Peters, but I think this is the clearest way to express the idea in English - with directly for added clarity if you don't mind the extra word.

the parts that are no supported are hanging since they are "unten" which means down or underneath.....this fits with the resto f the textm since they mention that this might generate swelling.
