The complete phrase is "Cartorio do Registro Civil das Pessoas Naturais e Anexo de Notas". I know that this means "Notary of the Civil Registry of Inidividuals" but what is the "Anexo de Notas"????

Though I'm not very fond of legal translations, I think "Cartório do Registro Civil das Pessoas Naturais e Anexo de Notas" means something like this in English: "Notary of Civil Registry of Natural Persons and Registrar’s Office".
