Why not just the way it is called in both languages?
If it is meant for a patient, than smth. like "discharge summary" would be better. But then in Russian it also could be just "выписка", and it is not this way.

What we called 'epicrisis' when I worked as a doc in Argentina is what here in the US is called "Discharge Summary": a synthesis of what the patient had and had done during his/her hospitalization.

disagree  DrMike: I'll tell you why...because this term is used on a sheet given by the hospital. NO MEDICAL PERSONNEL WILL UNDERSTAND THIS TERM IN ENGLISH. As a medical professional, I strongly disagree with this terminology.
