it's a yearly census of prevalence

In most medical statistical reports, data is usually presented as the evaluated items increase (or diminish ) in prevalence in the studied universe. Thus, EPINE, is a study of the prevalence of nosocomial (hospitals) infections in Spain. Conditions for inclusion in the study are described therein.

it is an annual prevalence rate/figure

In a public health context, 'prevalence' is usually a ratio representing an annual "snapshot" of the number of cases "on the books" (diagnosed and being followed) during a particular year relative to the population at risk, but the period can also be shorter or longer than a year, so the emphasis here is on the time period being considered.

It is not a 'census' - prevalence is calculated based on reports already on file, whereas a census is proactive and seeks out cases to enumerate.

Here's one definition of 'prevalence':
(epidemiology) the ratio (for a given time period) of the number of occurrences of a disease or event to the number of units at risk in the population

Looking at this again, I now propose (changing the order a bit):

It's a cross-sectional study/survey of annual prevalence
It’s a cross-sectional annual prevalence study/survey

Given the phrasing that follows - "se desarrolla una vez al año (Mayo)”,”Incluye sólo hospitales de agudos...”, I think you need the reference to a study or survey.

And you might want to say “it was” instead of “it is” - depending on when your text was written – since the EPINE project seems to be over.

The reason I suggest a change in my previous word order is this:
"la realización de estudios nacionales de prevalencia anual" (see ref above)
(studies plural, "prevalencia anual" hangs together, with annual modifying prevalence, not studies)

neutral Muriel Vasconcellos : Though the EPINE project apparently does conduct a survey, I don't think that's what the text is saying here. My interpretation is that the author is emphasizing that it's an annual figure, since prevalence can be a period of any length.

In the context of “se desarrolla una vez al año (Mayo)”,”Incluye sólo hospitales de agudos...”, I think it refers to a study or survey. A rate or figure couldn't be carried out once a year. Good point that "annual" should modify "prevalence".
