Basic clinical examination
This term appears in the "first impressions" part of a clinical examination of a patient who has just arrived in A&E with a history of haematemesis and melaena, before any examination of the systems. It goes along with the vital signs, BP, HR, temperature, oxygen sats, state of hydration and so on. There are hits on Google but none leave me any the wiser. I shall feel a fool when hundreds of you put me in the picture!!

Sounds like a Brit made the note ( "conscious and oriented times three" ? ). In the US. the medical shorthand would be AOx3 which
is alert and oriented times three ( in the three planes of consciousness : person, place, time ), which is to say, "consciente/alerto y orientado en tres planos
de consciencia ( persona, lugar, tiempo )".
