I am translating a medical report (psychiatry) from Puerto Rico. In the headings(title) of the report, there is the expression: Region de Arecibo.
Does anybody know how this could be translated into English (USA)

The complete heading is: CORPORACION DEL FONDO DEL SEGURO DEL ESTADO
REGION DE ARECIBO
EVALUACION INICIAL DE CONDICION EMOCIONAL

The rest of the document is a check list with spaces for the psychiatrist to fill up as a report.
